摘 要:目的 比较不同浓度碳酸氢钠溶液用于重度吸入性损伤患者气道护理的效果.方法 选取重度吸入性损伤并行呼吸机辅助呼吸的患者20例,采用随机数字表法分为常规组和观察组各10例.患者伤后第3天开始行气道冲洗,持续至伤后2周.常规组采用1.25%碳酸氢钠溶液冲洗气道,观察组采用5%碳酸氢钠溶液冲洗气道,每2小时1次,每次用量3~5 mL,无菌技术吸痰,痰液增多或氧饱和度下降时增加冲洗次数.结果 观察组患者主观感觉气道舒适度、痰液稀释度评分显著低于常规组(均P<0.05);两组痰培养真菌阳性率比较无差异.结论 5%碳酸氢钠溶液冲洗气道可有效稀释痰液,促进痰液引出,提高患者气道舒适度.Objective To investigate the effect of NaHCO3 in two different concentrations on airway care of severe inhalation injury patients. Methods Twenty severe inhalation injury patients on mechanical ventilation were randomly divided into two groups: the control group (1.25 % NaHCO3 used for airway irrigation) and the observation group (5 % NaHCO3 used). Airway irrigation was conducted three days after injury and lasted for two weeks, once per two hours, 3-5 mL each time; thereafter, sputum was suc- tioned. Besides, more irrigation was conducted when sputum increased or oxygen saturation decreased. Results Subjective score of airway comfort feeling and sputum viscosity in the control group was significantly lower than those in the observation group (P〈 0.05 for both), while there was no significant difference in rate of positive fungal cultures between the two groups (P〉0.05). Conclusion 5 % NaHCO3 can effectively dilute sputum and facilitate drainage, and enhance the degree of patientsrairway comfort.
