静脉泵注不同剂量右美托咪定在全麻术后寒战中的临床应用

摘　　要：目的 ：观察不同剂量右美托咪定用于治疗全麻术后寒战的临床效果、安全性和不良反应。方法 ：ASAⅠ-Ⅱ级全身麻醉下行腹部手术后在恢复室出现4级寒战的患者60例,随机分为A、B、C 3组（每组20例）：静注右美托咪定A组0.25μg/kg、B组0.5μg/kg、C组1μg/kg,3组患者均于10 min内输完。观察各组患者的寒战转归情况;各组患者用药前后心率、血压的变化;观察用药后不良反应,包括恶心、呕吐、口干、呼吸抑制（呼吸〈10 min-1或血氧饱和度〈90%）、心动过缓（心率〈50 min-1）。结果：A组治疗4级寒战显效率显著低于B组和C组（P〈0.05）。各组患者寒战发生后血压、心率均不同程度增高,各组用药后血压、心率显著下降（P〈0.05）。结论：静脉泵注右美托咪定0.5μg/kg治疗全麻术后寒战是安全可靠的。Objective： To observe the effects and safety and adverse reactions of different doses of dexmedetomidine being used for the treatment of postoperative anesthesia shivering. Methods： Sixty general anesthesia for abdominal surgery patients after four shivering in the recovery room, with ASA grades Ⅰand Ⅱ, were randomly divided into three groups（twenty cases each group）： intravenous dexmedetomidine group A given 0.25 μg/kg, group B 0.5 μg/kg, group C 1 μg/kg, three groups patients were injected within 10 min. To observe the chills outcomes and heart rate and the changes in blood pressure of patients in each group before and after treatment;to observe the adverse reactions after treatment, including nausea,vomiting, dry mouth, respiratory depression（RR10 min-1or SpO290%）, bradycardia（HR50 min-1）. Results： The four chills significant efficiency was significantly lower in group A than those in group B and group C（P0.05）. The occurrence of chills and the blood pressure and heart rate at patients in each group were increased to various, each group after treatment of blood pressure, heart rate decreased significantly after treatment（P0.05）. Conclusion： Intravenous 0.5 μg/kg dexmedetomidine infusion treating shivering after general anesthesia is safe and reliable.
