联合检测血清CysC、血清hsCRP、和尿β2-MG在2型糖尿病早期肾病中的诊断价值

摘　　要：目的探讨血清胱抑素C(CysC)、血清超敏C反应蛋白(hsCRP)、尿微量白蛋白(U-mAlb)和尿β2-微球蛋白(β2-MG)对诊断早期糖尿病肾病(DN)的应用价值。方法根据尿微量白蛋白水平将70例DN患者分为尿蛋白正常组(A组)20例,尿蛋白少量组(B组)27例,尿蛋白大量组(C组)23例,另外选取健康对照组30例,同时检测各组患者的血清CysC、hsCRP、尿β2-MG和U-mAlb水平。结果与A组和对照组比较,B组和C组各项指标水平有显著统计学差异(P<0.01),A组与对照组比较血清CysC、hsCRP水平差异有统计学意义(P<0.05),DN组上述四项指标水平各组间存在显著性差异(P<0.01)。结论联合检测血清CYS-C、hsCRP、U-mAlb和尿β2-MG水平可作为DN早期肾损伤敏感指标,全面协助DN的早期诊断。Objective To investigate the diagnosis value of serum cystain C（Cys-C）,high sensitive C-reactive pro-tein（hsCRP）,urinary albumin（U-mAlb）and Urineβ2 - microglobulin（β2-MG ）in the early diabetics nephropathy. Methods According to urinary album level,the 70 patients with diabetes were divided into three groups：the normal al-buminuria group（the group A）20 cases,the micro albuminuria group（the group B）27 patients,and the clinical proteinu-ria group（the group C）23 cases,and select the healthy control group 30 subjects.The levels of four groups＇s Cys-C, hsCRP,U-mAlb,β2-MG were detected.Results Compared with the group A and control group,the levels of four indi-cators of the group B and the group C both significantly increased （P 〈0.01）,the levels of Cys-C、hsCRP of the group A were significantly higher than the control group,（P 〈0.05）,The levels of four indicators were significantly higher in the diabetics nephropathy groups（P 〈0.01）.Conclusion Joint detection of serum Cys-C,hsCRP,U-mAlb and Urineβ2-MG levels as early renal injury sensitive indicators and full assistance to the early diagnosis of diabetics nephropathy.
