性激素水平对老年人高血压病肾气亏虚证证型诊断客观化影响的研究

摘　　要：目的：探讨性激素平衡紊乱与老年人高血压病肾气亏虚证发病的相关性，为老年人高血压病肾气亏虚证证型诊断客观化的研究提供依据。方法收集年龄65岁～75岁患有1级、2级高血压病，并符合肾气亏虚证或肝火上炎症辨证标准患者，每组40例；选择理化检查均正常的健康老年人作为正常对照组，男性20例，女性20例。对以上3组人群进行对照研究，采用放射免疫分析法比较3组受试者性激素水平的差异。结果与健康老年人和肝火上炎证患者相比，老年人高血压病肾气亏虚证患者存在性激素平衡紊乱的情况：老年男性高血压病肾气亏虚证患者存在雄激素（T）水平下降（P〈0.05）、T/E2比值显著下降（P〈0.01）现象；老年女性高血压病肾气亏虚证患者存在雌激素（E2）水平下降（P〈0.05）、T/E2比值上升（P〈0.05）的现象。结论性激素平衡失调是老年人高血压病肾气亏虚证发生的危险因素，可作为其病因的推测及客观化证型诊断的参考。Objective To explore the relationship between sex hormone balance disorders and the elderly hypertension disease kid-ney with qi deficiency syndrome.Methods There were 40 elderly patient with hypertension in kidney deficiency syndrome group （male 20,female 20）,40 elderly patients with hypertension in inflammation syndrome group （male 20,female 20）,and 40 subjects as normal controls.Sex hormones of all patients were collected and compared by ria analysis.Results The level of androgen T （P〈0.05）and T/E2 ratio （P〈0.01）were significantly decreased in elderly male patients with hypertension and kidney deficiency syndrome.The estrogen E2 level was reduced （P〈0.05）and T/E2 ratio rose （P〈0.05）in female patients with hypertension and kidney qi deficien-cy syndrome.Conclusion Sex hormone level disorder was risk factors in elderly hypertension with kidney qi deficiency syndrome.
