麻醉方式对高龄股骨粗隆间骨折患者术后恢复效果的影响分析

摘　　要：目的:对比分析全身麻醉与椎管内麻醉对高龄股骨粗隆间骨折患者术后恢复效果的影响。方法:选取2011年2月-2013年4月笔者所在医院收治的50例高龄股骨粗隆间骨折患者,采用随机数字表法将其分为全身麻醉组和椎管内麻醉组,各25例。对两组患者术后并发症、住院时间及下地活动时间等恢复情况进行对比分析。结果:所有患者术中骨折部位得到较好的复位与固定。椎管内麻醉组发生术后并发症3例(12.0%),全身麻醉组术后并发症10例(40.0%),两组比较差异有统计学意义(P<0.05)。此外椎管内麻醉组下地活动时间及住院时间明显优于全身麻醉组,差异有统计学意义(P<0.05)。结论:椎管内麻醉对患者术后恢复效果(术后并发症、下地活动所需时间及住院时间)影响优于全身麻醉组。麻醉方式对高龄股骨粗隆间骨折患者术后恢复效果有一定的影响,在采取PFNA髓内钉固定术治疗时,可优先选择椎管内麻醉方式。Objective：Comparative analysis of general anesthesia and spinal anesthesia for surgery among patients intertrochanteric fracture recovery effect of impact.Method：50 cases from February 2011 to April 2013,admitted in the hospital of intertrochanteric fracture in aged patients,using the random number table method which can be divided into general anesthesia group and spinal canal anesthesia group,25 cases each. On two groups of patients with postoperative complications,the length of time and engaged in activities such as recovery were analyzed.Result：All of the patients got good reduction and fixation of intraoperative fracture position. Spinal canal anesthesia group of postoperative complications occurred in 3 cases（12.0%）,10 cases（40.0%） of postoperative complications of general anesthesia group,between the two groups there was statistically significant（P〈0.05）. Moreover intra-spinal canal anesthesia group was obviously better than the down time and hospital stay general anesthesia group,the difference was statistically significant（P〈0.05）.Conclusion：Spinal canal anesthesia for patients with postoperative recovery effect（postoperative complications,and down time needed for activities and the length of time） effect is better than that of general anesthesia group. Anesthesia on postoperative recovery of intertrochanteric fracture in aged patients effect have a certain influence,when treated with PFNA intramedullary nail fixation,give priority to choose the way to vertebral tube anesthesia.
