运用德尔菲法对《冠心病稳定型心绞痛脾虚痰浊证·中医证候计分表》条目筛选的研究

摘　　要：目的:(1)筛选、优化条目并制定《冠心病稳定型心绞痛脾虚痰浊证·中医证候计分表》。(2)建立能体现中医药治疗冠心病稳定型心绞痛脾虚痰浊证的疗效评价工具。方法:在参考国标、行标的基础上,结合专家会议意见形成《冠心病稳定型心绞痛脾虚痰浊证·中医证候计分表》,运用德尔菲(Delphi)法向行业内50位专家发送2轮调查问卷,并对问卷调查结果进行统计分析。结果:调查问卷收回率100%,专家积极系数为100%,专家总体权威系数为85%,专家意见的总体协调系数W为0.296,P<0.05。共删除4个条目,确定将胸闷、胸痛、脘痞、便溏等16个条目及其轻重程度纳入中医证候计分表中。结论:参与调查的专家代表性和积极性均较高,其权威程度以及协调性较好。应用德尔菲法对《冠心病稳定型心绞痛脾虚痰浊证·中医证候计分表》条目的筛选达成一致的专家共识。但是,仍需要进一步的临床实践来评价该证候计分表对于冠心病稳定型心绞痛病证结合疗效评价的作用价值。Objective ： 1. To screen and optimize the item pool of TCM syndrome score table of spleen deficiency and phlegm turbid syndrome of stable angina pectoris. 2. To establish evaluation tool which could reflect the efficacy of Chinese medicine treating spleen deficiency and phlegm turbid syndrome of stable angina pectoris. Methods ： In reference to the national standard and the professional standard,we combined with expert advice in order to form the TCM syndrome score table. The Delphi method was used and 2 round questionnaires were sent to cardiovascular specialists. The survey results were analyzed statistically. Results ： The questionnaire recovery rate was 100%. Expert positive coefficient was 100%. Expert authority coefficient was 85%. Expert advice for overall coordination coefficient W was 0. 296 （P 〈 0.05 ）. Items such as chest pain, chest swelling,loose stools and other 12 i- tems were brought into its TCM syndrome score table after 4 entries were deleted. Conclusion： The representation and enthusiasm of experts participating in the survey were high, as well as the extent of its authority and coordinate. The application of the Delphi method for screening items of TCM syndrome score table of spleen deficiency and phlegm turbid syndrome of stable angina pecto ris could reach experts consensus. However, further clinical practice is still need to evaluate the value of the score table in evaluating TCM efficacy treating stable angina pectoralis.
