针刀松解法治疗习惯性髌骨脱位的临床研究

摘　　要：目的:观察针刀松解术治疗习惯性髌骨脱位(RSP)的临床疗效,及其对相称角等指标的影响,并探讨针刀松解术RSP的作用机制及其对膝关节功能的影响。方法:将符合纳入标准的76例患者随机分为治疗组和对照组,治疗组(38例)采用针刀松解法治疗,对照组(38例)采用针刺推拿治疗。两组均治疗2周。观察患者临床症状、体征、膝关节X线(相称角、股骨髁间角、髌骨倾斜角)。根据Lysholm和Gillquist膝关节评分标准进行系统评估分析疗效。结果:治疗组治疗后临床症状、体征、膝关节X线等均有不同程度改善,与治疗前比较差异有统计学意义(P<0.05);患者相称角、股骨髁间角、髌骨倾斜角均有改善,与治疗前比较差异显著,有统计学意义(P<0.05)。结论:患者治疗后疼痛、关节活动功能均得到明显改善,根据病因及生理解剖改变选择针刀松解治疗习惯性髌骨脱位可取得满意的效果,具有创伤小、恢复快的特点,值得进一步推广应用。Objective： To observe the curative effect of acupotomy operation in the treatment of habitual dislocation of patellar（RSP） , and its effect on the match angle indicator and to investigate the knife loosening mechanism solution of RSP and its influence on knee function. Methods：76 patients who met the criteria were randomly divided into treatment group and control group. The treatment group （38 cases）was treated by acupotomy lysis,knife one time a week. The control group （38 cases）was treated by acupuncture and massage therapy. Two groups were treated for two weeks. Observe patients with clinical symptoms, signs, knee X -ray（ appropriate angle ,femoral intercondylar angle, patellar tilt angle ）. Results：Clinical symptoms, signs, knee X -ray in the treatment gwup bad different degrees of improvement compared with those before treatment. The difference was statistically significant （P 〈 0.05 ）. Patients with femoral condyle angle, angle, patellar tilt angle could be improved compared with those before treatment, and there was a statistical significance （ P 〈 0.05 ）. Conclusion ： After treatment, the pain, the joint function were significantly improved. According to the cause and physiological anatomy, acupotomy treatment for habitual patellar dislocation can achieve a satisfied effect, with small trauma, quick recovery, which is worthy of further popularization and application.
