中心静脉导管注入尿激酶治疗结核性包裹性胸腔积液

摘　　要：目的分析留置中心静脉导管注入尿激酶应用于结核性包裹性胸腔积液的临床治疗效果。方法选取2011年8月～2012年11月海丰县彭湃纪念医院60例结核性包裹性胸腔积液患者，随机均分成对照组和试验组（n=40）。对照组患者给予穿刺抽液治疗，试验组患者给予中心静脉导管持续引流注入尿激酶治疗。对2组患者的疗效进行对比分析。结果试验组患者的胸腔积液引流量（2143±221）mL显著多于对照组（1365±214）mL（P〈0.05）；胸膜厚度（2.76±0.23）mm显著低于对照组（3.25±0．31）mm（P〈0.05）；试验组FVO％为（93.2±3.2），FEV，％为（922±4．1）高于对照组的（86．5±3．8）、（84．3±3、4）（P〈0、05）。试验组患者的总有效率为92．5％，对照组为80.0％，试验组疗效优于对照组（P〈0．05）。结论中心静脉导管持续引流注入尿激酶用于结核性包裹性胸腔积液的临床治疗效果佳，患者的肺部功能得到有效的改善，且不良反应少，安全性高。Objective To analyze clinical effect of the indwelling central venous catheter injection of urokinase used in tuberculous pleural effusion parcel. Methods 80 cases of tuberculous pleural effusion in the hospital from August 2011 to November 2012 were randomly divided into two groups, control group of 40 patients were treated with puncture fluid therapy, experimental group of 40 patients were treated with continuous central venous catheter drainage urokinase therapy. The efficacy of two groups of patients were compared. Results The patient＇s pleural effusion drainage in experimental group was （2143±221）mL significantly more than the control group （1365±214）mL （P〈0.05）; Pleural thickness in experimental group was （2.76±0.23）mm significantly lower than that in the control group （3.25±0.31 ）ram （P〈0.05）; FVC% in experimental group was （93.2 ± 3.2）, FEV EEV1% was （92.2 ±4.1） ,higher than that in the control group （86.5＋3.8） and （84.3±3.4） （P〈0.05）. The total effective rate in experimental group was 92.5% ,while that was 80.0% in the control group, the therapeutic effect in the experimental groupwas better than the control group （P〈0.05）. Conclusion Central venous catheter for continuous drainage urokinase encapsulated tuberculous pleural effusion can achieve good effect, the patient＇s lung function has been effectively improved, and has fewer adverse reactions, safe.
