摘 要:目的探讨通过控制和改善口腔微环境,降低胶质瘤术后化疗患者口腔感染发生率的可行性,达到改善免疫功能、提高化疗缓解率。方法选择2010年1月-2011年12月进行胶质瘤术后化疗的患者90例,随机分成对照组46例和研究组44例,其中对照组患者采用常规术后干预,研究组在对照组常规干预的基础上,采用测量口腔唾液pH值,根据pH值选择含漱液、针对口腔的具体情况进行健康教育和改善口腔微环境,观察两组患者口腔黏膜感染率和评分,并进行比较。结果口腔黏膜感染率对照组为56.52%,研究组为20.45%,研究组口腔黏膜感染率显著降低,差异有统计学意义(P<0.05);干预后口腔溃疡评分对照组(1.93±0.43)分,研究组为(0.34±0.05)分,研究组口腔溃疡评分明显降低,差异有统计学意义(P<0.05)。结论通过控制改善口腔微环境,可有效降低胶质瘤术后化疗患者的口腔感染发生率和减轻口腔黏膜溃疡,值得在临床大力推广。OBJECTIVE To evaluate the feasibility of controlling and improving oral microenvironment for reducing the incidence of oral infections on patients who accepted gliomas postoperative chemotherapy, so as to improve immunologic function and increase remission rate of chemotherapy. METHODS A total of 90 patients who accepted glioma postoperative chemotherapy in our hospital from Jan. 2010 to Dec. 2011 were selected and randomly divided into the two groups including the control group (46 cases)and the study group (44 cases) and all patients in control group were treated with the routine nursing, all patients in study group with special nursing based on the routine nursing, including sampling their saliva in mouth and measuring pH value, and according to the pH value of saliva, appropriate mouthwashe was chosen, and according to the specific circumstances of oral, health education and controlling oral microenvironment were given. The patients/ oral mucosal infection rate and scores were observed and compared in both groups. RESULTS The oral mucosal infection rates were 56. 52% and 20.45% in control group and study group, and the rate was significant lower in study group(P 〈0.05). After intervention treatment, the oral ulcer score of the control group was (1. 93 ± 0.43), study group was (0. 34±0.05), and there was significantly lower in study group (P〈0.05). CONCLUSIONS It can effectively reduee the incidence of oral infections and relief the oral ulcer of patients who accepted glioma postoperative chemotherapy by eontrol ling and improving oral microenvironment. It is worth to be promoted in clinical.
