PDCA循环法在手卫生依从性管理中的应用

摘　　要：目的了解医务人员手卫生依从性现状,寻找存在的问题及影响因素,逐步采取有效措施,以提高医务人员手卫生依从性。方法采取PDCA(计划-实施-检查-处理)循环管理模式,进行手卫生持续质量改进,加强全员培训,完善手卫生设施,定期检查、分析、反馈与总结。结果实施PDCA循环后,手卫生依从率医师由实施前的45.45%上升至实施后86.96%;护士由72.55%上升至94.55%,差异有统计学意义(χ2=51.58,P<0.01;χ2=38.84,P<0.01),PDCA实施前后护士的手卫生依从率均高于医师。结论应用PDCA循环法进行持续质量改进,可促进医务人员手卫生依从性的提高。OBJECTIVE To investigate the situation of hand hygiene compliance of medical personnels and find the problems and influencing factors, in order to improve hand hygiene compliance of medical personnels by taking effective measures step by step. METHODS PDCA （Plan-Do-Check-Action） circular management mode was performed to continuously improve hand hygiene compliance of medical professionals by strengthening trainings to all staffs, improving hand hygiene relative facilities, and inspecting-analyzing-feedback-summarizing in period. RESULTS The hand hygiene compliance was enhanced from 45.45% to 86.96% and from 72.55% to 94.55% for doctors and nurses respectively after PDCA management was performed, and the differences were significant（Х^2 = 51.58,P〈0.01;Х^2 =38.84,P〈0.01）. The compliances of nurses before or after PCDA management were both higher than those of doctors. CONCLUSION The application of PDCA management for continuous quality improvement could obviously enhance hand hygiene compliance of medical personnels.
