一侧扪不到睾丸的儿童隐睾症的诊断与治疗

摘　　要：目的　探讨一侧扪不到睾丸的儿童隐睾症的临床特点及处理方法。方法　对 42例一侧扪不到睾丸的患儿行手术探查及治疗。结果　 42例中 2 8例为高位隐睾 ;2例已降入阴囊 ,2例位于股管内 ,1例位于腹股沟管内 ,均发育不良 ;9例为患侧睾丸缺如。行睾丸固定术 2 8例 ;切除发育不良 ,异位睾丸 9例 ;仅作探查 5例。 2 5例获随访半年至 11年 ,术后睾丸回缩 3例 ,萎缩 2例。结论　一侧扪不到睾丸的患儿 ,其睾丸大多位于腹腔内 ,其次为患侧睾丸缺如、发育不良、异位或已降入阴囊伴睾丸极度发育不良。术前仍缺乏可靠的确诊方法 ,手术探查仍然是最可靠的诊断及治疗方法。Objective To investigate the clinical characteristics and management of unilateral impalpable testis.Methods Exploratory operations were performed for 42 cases of unilateral impalpable testis admitted into this hospital from may 1988 to June 2000.Results In 28 cases,the testis were highly located,of which 26 were located in the peritoneum and 2 in the retroperitoneum.Two testis already descended into the scrotum,2 migrated to the femoral canal,and one lay in the inguinal canal;all of these testis were found dysplastic.In the rest 9 cases,no testis was found on the diseased side in the exploratory operation.Twenty eight cases received orchidopexy,9 cases received surgical removal of dysplastic or ectopic testis,and 5 cases just underwent exploration of testis.Twenty five cases were followed up for half a year to 11 years,retraction of operated testis was found in 3 cases,and atrophy in 2 cases.The two testis,which had descended into the scrotum and not been treated during the exploration,remained undeveloped.Conclusion Most of unilateral impalpable testes were located in the peritoneum.The other conditions occasionally encountered were monorchism,dysplastic testis,ectopia or the testis had descended into the scrotum but remained undeveloped.It is important to be familiar with these characteristics during the exploratory operation.Since no other reliable method is available to detect the testis in each case preoperatively, early exploratory operation is still the most reliable diagnostic and treatment method.
