巨噬细胞CD40L表达在腹膜炎中的临床意义

摘　　要：目的 探讨巨噬细胞ＣＤ４０配体（ＣＤ４０Ｌ）表达与持续性不卧床腹膜透析（ＣＡＰＤ）腹膜炎发生发展的关系。方法 运用流式细胞仪观察ＣＡＰＤ患者急性腹膜炎时腹透透出液中巨噬细胞ＣＤ４０Ｌ表达，并与ＣＡＰＤ非腹膜炎患者比较。结果 急性腹膜炎发生时，透出液中表达ＣＤ４０Ｌ阳性巨噬细胞数显著增多，巨噬细胞表达ＣＤ４０Ｌ水平显著增加。与非腹膜炎患者比较有显著差异（Ｐ＜０．０５，Ｐ＜０．０１）。急性腹膜炎缓解后．患者透出液中表达ＣＤ４０Ｌ阳性巨噬细胞数及巨噬细胞表达ＣＤ４０Ｌ水平均较发作期显著降低（Ｐ＜０．０５）。结论ＣＡＰＤ腹膜炎时巨噬细胞高表达的ＣＤ４０Ｌ对于限制腹膜炎的发生发展具有积极意义。Objective To investigate the possible regulation of CD4O ligand(CD4OL) expression in peritonitis during continuous ambulatory peritoneal dialysis( CAPD) . Methods CD4OL expression on macrophages from peritoneal dialysate were detected by FACS analysis. Results CD4OL expression on macrophage increased significantly in peritonitis group(4. 67 ±6. 22) as compared to the control group (0.75 ±0. 29). P < 0.01. The CD4OL positive cells increased significantly as well during peritonitis (97.84±1.62) in the peritonitis group as compared to in the control group(73. 25 ±5.44). P < 0.05. After successful treatment, the expression of CD4OL was significantly reduced(3. 76 ±1. 02 vs 1. 04± 0.02 P < 0. 05) . Conclusion Functional expression of CD4OL on macrophages may take part in acute inflanunatory response during CAPD peritonitis and may play an important role in the local defense against infection in the peritoneal cavity.
