摘  要:目的:探讨孕中、晚期胎儿纤维蛋白(fFN)、人绒毛膜促性腺激素(hCG)及孕妇宫颈长度在早产预测中的价值。方法:抽取2012年5月-2013年12月绵阳市妇幼保健院接收的160例先兆早产孕妇,取宫颈阴道分泌物经ELISA法检测fFN、hCG水平,采用阴道B超对宫颈长度进行测量,对比单独检测指标与三项联合检测指标的早产预测结局。结果:fFN检测阳性孕妇早产率为84.2%,hCG检测阳性孕妇早产率为71.7%,宫颈长度〈25mm孕妇早产率为83.3%,宫颈长度≥25mm孕妇早产率为12.5%,fFN与hCG阳性检测联合宫颈长度预测孕妇早产率为97.2%,联合检测的平均敏感度、平均特异度、阳性率、阴性率依次为98.6%、60.4%、95.9%、99.7%。结论:孕中、晚期fFN、hCG联合孕妇宫颈长度检测在早产预测中价值显著,临床应用价值较高。Objective: To explore the values of Fetal fibronectin (fFN) , human chorionic gonadotropin (hCG) and maternal cervical length during the second and the third trimesters of pregnancy on premature labor. Methods: A total of 160 pregnant women with threatened premature labor treated in the hospital from May 2012 to December 2013 were selected, vaginal secretion samples were obtained to detect fFN and hCG levels by ELISA. Cervical length was measured by transvaginal ultrasonography; the pregnancy outcomes of single detection and joint detection of three indexes were compared. Results: The incidence rates of premature labor in pregnant women receiving single detection of fFN, hCG, cervical length 〈25 mm and cervical length≥25 mm were 84. 2%, 71.7%, 83.3%and 12.5%, respectively; the incidence rates of premature labor in pregnant women receiving joint detection of fFN and hCG was 97.2% ; the mean sensitivity, mean specificity, positive rate and negative rate of joint detection were 98.6%, 60. 4%, 95.9% and 99. 7%, respectively. Conclusion: The values of fFN, hCG and maternal cervical length during the second and the third trimesters of pregnancy on premature labor are significant, and the clinical application value is relatively high.
