摘 要:目的:探讨早产儿贫血发生的影响因素。方法:选择115例发生贫血的早产儿,按胎龄分为胎龄〈32周组,32~35周组,35—37周组,比较3组早产儿出生体重、出生时基础Hb值、贫血发生时间、贫血时Hb值和最低Hb值等方面有无差异;按照贫血发生时间分为早期贫血组和晚期贫血组,分析两组早产儿在胎龄、出生体重、出生时基础Hh值、采血量、溶血出血史、母亲贫血史等方面有无差异。结果:3组早产儿在出生体重、出生时基础Hb值、贫血发生时间、贫血Hb值和最低Hb值等指标差异有统计学意义(P〈0.05);早期贫血组和晚期贫血组在胎龄、出生体重方面差异无统计学意义(P〉0.05),出生时基础Hb值、初始发现贫血时Hh值、采血量、溶血出血史和母亲贫血史差异有统计学意义(P〈0.05)。结论:早产儿贫血与胎龄、出生体重、基础Hb值、溶血和失血史、母亲贫血史、住院期间抽血量等因素紧密相关。Objective: To explore the relevant influencing factors of anemia in premature infants. Methods : A total of 115 premature infants with anemia were selected and divided into gestational age 〈 32 week group, 32 - 35 week group and 35 - 37 week group according to gestational age, birth weights, basic hemoglobin levels at birth, onset times of anemia, hemoglobin levels during anemia and the lowest hemoglobin levels in the premature infants of the three groups were compared ; the premature infants were divided into early anemia group and late anemia group according to the onset time of anemia, the gestational ages, birth weights, basic hemoglobin levels at birth, the amounts of blood drawing, the histories of hemostasis and maternal anemia in the two groups were compared. Results: There were statistically significant differences in birth weight, basic hemoglobin level at birth, onset time of anemia, hemoglobin level during anemia and the lowest hemoglobin level among gestational age 〈 32 week group, 32 - 35 week group and 35 - 37 week group ( P 〈 0. 05 ) ; there was no statistically significant difference in gestational age and birth weight between early anemia group and late anemia group ( P 〉 0. 05 ) , there were statistically significant differences in basic hemoglobin level at birth, hemoglobin level when detecting anemia, the amount of blood drawing and maternal anemia history between early anemia group and late anemia group ( P 〈 0. 05 ) . Conclusion: Anemia of premature infants is closely correla- ted with gestational age, birth weight, basic hemoglobin level, hemolysis and blood loss histories, maternal anemia history and the amount of blood drawing in hospital.
