高位右心房游离壁起搏三年随访研究

摘　　要：目的:研究高位右心房主动电极植入的长期效果和对临床的影响。方法:选择因病态窦房结综合征或者房室传导阻滞在我科植入双腔起搏器、心房主动电极定位于高位右心房游离壁的患者作为研究对象,并将同期被动电极植入右心耳的患者作为对照,比较两组的临床特点、起搏不良事件及对心脏结构功能的影响。结果:在主动电极组随访期显著长于被动电极组的情况下,两组间心房内径、左室射血分数无差异,其术前、术后变化亦无差异;仅主动电极植入时阻抗高于随访期(P<0.05),其阈值、P波高度等均无差异;不良事件发生率亦无显著差异。结论:高位右心房起搏长期治疗安全有效,操作相对简单,手术时间短,可以作为心房起搏的一个可行的替代位点。Objective： To study the long-term efficacy of pacing with screw-in lead from free wall of high right atrium（ HRA）. Methods： Patients with SSS or AVB,were divided to two groups： those who accepted screw-in lead were in experimental group and those who accepted passive lead in right atrial appendage were in control group. Threshold,P wave amplitude,impedance,heart function,and rate of adverse event of the tow groups were compared afterwards. Results： Experimental group had longer following-up time than control group. However there was no significant statistic difference of threshold,P wave amplitude,impedance,heart function,and the adverse event in the two groups. The impedance of experimental group was high at the end of implantation,then decreased during the follow-up period（ P 0. 05）. Conclusion： Pacing from free wall of HRA is considered safe and efficacious. The procedure is feasible and needs less time. It is thus a safe alternative pacing site.
