复方四逆温胆汤对中风后疲劳患者血清炎症因子及疲劳程度的影响

摘　　要：目的：通过观察复方四逆温胆汤联合康复训练，治疗中风后疲劳的临床实践，观察疲劳指标及血清炎症因子的变化，为选择最佳的中风后疲劳的治疗方案提供依据。方法：将符合本研究条件的90例中风后疲劳患者随机分为对照组、复方四逆温胆汤组、复方四逆温胆汤＋康复训练组，分别治疗1个月。比较3组患者及治疗前后疲劳指标及血清炎症因子的变化及其相关性。结果：①治疗后3组的SS—QOL、FSS、VAS评分均下降，但复方四逆温胆汤＋康复训练组下降最明显（P〈0．05）；②3组患者治疗前的血清因子水平基本一致（P〉0．05）；治疗后3组血清因子水平均有不同程度下降，但复方四逆温胆汤＋康复训练组下降最明显，变化趋势和疲劳程度基本一致；③疲劳指标与炎症因子水平呈正相关关系，相关系数r为0．330～0．567，有统计学意义（P〈0．05）。结论：复方四逆温胆汤治疗中风后疲劳，可明显改善患者的自觉疲劳感觉，降低患者体内的炎性反应，配合现代康复训练，治疗效果更佳，可在临床推广使用。Objective ： To observe the effect of compound Sini Wendan Decoction ＋ rehabilitation training for the treat- ment of post - stroke fatigue and to provide a basis for the selection of the best post - stroke fatigue treatments. Methods ： 90 post - stroke fatigue patients were randomly divided into the control group, the compound Sini Wendan Decoction group, the compound Sini Wendan Decoction ＋ rehabilitation training group and be treated for one month. The fatigue index and changes in serum inflammatory factors were analyzed. Results：（1）After treatment, the SS -QOL, FSS and VAS scores of the three groups were decreased, but Sini Wendan Decoction ＋ rehabilitation training group＇ s were decreased most significantly （ P 〈 0.05 ）. （2）The serum cytokine levels in three groups of patients after treatment were decreased, but those of the group C decreased most obviously. （3）The fatigue level and inflammatory factors were correlated and the correlation coefficient r = 0.330 -0. 567 with statistical significance （P 〈 0. 05 ）. Conclusions：The compound Sini Wendan Decoction treating post - stroke fatigue can significantly improve the patient＇s self- feeling of fatigue, reduce the inflammatory response and combined with modern rehabilitation training can reach better effect.
