27例子痫前期合并HELLP综合征临床分析

摘　　要：目的分析27例子痫前期合并HELLP综合征的临床特点及对母儿的影响,探讨该疾病的诊治方法。方法对27例子痫前期合并HELLP综合征患者的临床资料进行回顾性分析,比较两组的临床表现、实验室指标、围产期并发症及分娩结局。结果完全性HELLP组的发病孕周(33.7±3.1)周、分娩孕周(36.2±3.1)周,部分性HELLP组的发病孕周(34.1±3.7)周、分娩孕周(35.1±3.2)周,差异无统计学意义(P>0.05);完全性HELLP组血小板计数(62.4±21.9)U/L明显低于部分性HELLP组(86.1±30.8)U/L(P<0.05),乳酸脱氢酶(645.8±268.0)U/L则显著高于部分性HELLP组(363.3±173.2)U/L(P<0.05);丙氨酸转氨酶、天冬氨酸转氨酶在两组间的比较差异亦存在统计学意义(P<0.05)。结论 HELLP综合征是严重的产科并发症,尤其完全性HELLP综合征,如能早期诊断、早期积极治疗,母婴预后无显著差异。Objective To analyze the clinical characters and maternal-neonatal outcome of 27 cases preeclampsia with HELLP syndrome and to discuss on the methods of diagnosis and treatment of the disease. Methods Retrospective analysis of date among 27cases of patients of preeclampsia with HELLP syndrome. The clinical characters, blood test, complications and pregnant outcomes were analyzed. Results No significant difference was found between the two group in incidence of gestational age and gestational age of delivery（33.7±3.1）weeks vs（34.1±3.7） weeks, （36.2±3.1） weeks vs （35.1±3.2） weeks, P〉0.05. PLT count in complete HELLP group （62.4±21.9） U/Lwas significantly lower than that in partial HELLP group（86.1±30.8）U/L,（P〈0.05）. LDH value in complete HELLP group（645.8±268.0） U/Lwas significantly higher than that in partial HELLP group（363.3±173.2） U/L,（P〈0.05）. There were significant differences between the liver enzymes in complete HELLP group and partial HELLP group. Conclusion HELLP syndrome is severe obstetric complications, in particular, for complete HELLP syndrome patients. Early diagnosis and active treatment can make prognosis no significantly different.
