System evaluation on reinfusion of gastric retention fluid for nasal feeding patients

摘　　要：[目的]评价胃潴留鼻饲病人经胃管回输胃潴留量对胃潴留发生率及并发症的影响。[方法]计算机检索2000年1月—2013年4月Cochrane Database of Systematic Reviews、JBI Database of Systematic Reviews、Pubmed、EMbase、中国生物医学文献数据库(CBM)、中国期刊全文数据库(CNKI)、万方数据库纳入鼻饲病人胃潴留量管理的系统评价及随机对照试验文章,采用RevMan 5软件进行Meta分析,对于无法转换合并的数据,采用描述性分析。[结果]纳入2篇随机对照试验共140例病人,结果显示回输胃潴留液会增加堵管发生率,然而全部丢弃又易导致血清钾浓度降低从而增加潜在风险。[结论]尚无证据显示,回输胃潴留量能有效提高胃潴留管理并且不增加潜在并发症。需要进行更多设计严格的、多中心、大样本随机对照实验对胃潴留量处理效果进行验证。Objective:To evaluate the influence of gastric retention amount refused into nasal feeding patients via gastric tube on incidence and complications of gastric retention patients.Methods:Computer retrieval was carried out for systematic evaluation and randomized controlled trial papers on management of gastric retention amount of nasal feeding patients in Cochrane Database of Systematic Reviews,JBI Database of Systematic Reviews,Pubmed,EMbase,Chinese biomedical literature Database(CBM), China journal full-text Database(CNKI),Wanfang Database from January 2000to April 2013.RevMan 5software was used for Meta analysis,and descriptive analysis was carried out for data which could not be converted and merged.Results:2randomized controlled trial containing total 140patients were included,and the results showed that Reinfusion of gastric retention liquid would increase the incidence of plugging,but to discard all easily would lead to the serum potassium concentration reduced,and then increase the potential risks.Conclusion:There are no evidences to suggest that reinfusion amount of gastric retention can effectively improve gastric retention management and does not increase potential complications.The more strict,multicenter,large sample randomized controlled trials need to be designed for validation of treatment effect of gastric retention amount.
