摘 要:目的探讨一种改进的脊柱取活检方法对椎体病变诊断的价值。方法改进脊柱取活检方法,用经皮椎体成形术部分手术器械联合小号刮匙及髓核钳,在C臂X光机透视引导下采用经皮穿刺经椎弓根取活检的方法,对23例疑难椎体病变行活检术,8例同时行椎体后凸成形术。结果 23例均成功穿刺取材,穿刺活检的阳性率为100%,全部病例取得较好质量病理切片,获得确诊,未发生穿刺并发症。结论 C臂X光机透视引导下使用椎体成形术手术器械联合小号刮匙及髓核钳,经皮穿刺经椎弓根穿刺活检是一种微创、可靠、安全的的活检方法,并可同时行PKP手术治疗。Objective To discuss an improved spinal biopsy method to improve the diagnostic value of vertebral bodylesions. Methods With part of percutaneous vertebroplasty instruments and small curette and nucleus pulposus clamp,23 cases of complicated vertebral body lesions underwent C-arm imaging guided transpedicular biopsy. Meanwhile 8cases were treated by percutaneous kyphoplasty. Results 23 cases were successful puncture, positive rate of biopsy was100%, all of patients achieved good quality pathological slices,diagnosis,no puncture complications occurred.ConclusionC-arm imaging guided transpedicular biopsy with part of percutaneous vertebroplasty instruments and small curette andnucleus pulposus clamp is a minimally invasive, reliable and safe method of biopsy, and patient can be treated by percutaneouskyphoplasty meanwhile.
