血管内隔绝治疗胸主动脉瘤的临床效果观察

摘　　要：目的探讨血管内隔绝治疗胸主动脉瘤的临床效果。方法选取2009年6月至2013年5月收治的胸主动脉瘤患者48例进行分析,按照治疗方法分成观察组和对照组各24例,观察组采用血管内隔绝治疗,对照组采用开放手术治疗,观察两组手术情况、术后并发症发生率和病死率以及治疗前后生存质量评分。结果观察组手术时间、术中出血量和住院时间明显少于对照组(P<0.05),并发症发生率和病死率明显低于对照组(P<0.05);两组治疗后3个月生存质量评分明显提高(P<0.05),且观察组提高水平明显优于对照组(P<0.05)。结论血管内隔绝治疗胸主动脉瘤手术时间短,术中出血量少,患者恢复快,并发症和病死率低,能够提高患者的生存质量,中远期疗效好,效果显著。Objective To explore the clinical effect of isolated endovascular surgery in treatment of thoracic aortic aneurysm. Methods The clinical data of 48 patients with thoracic aortic aneurysm in this hospital during June 2009 to May 2013 were retrospectively analyzed, and they were divided into observation group and control group according to the procedures of operation, each with 24 patients. Patients in observation group were treated by isolated endovascular surgery, patients in control group were performed with open operation, and their postoperative morbidity of complications, mortality and scores of quality of life were assessed before and after treatment. Results The operating time, blood loss and duration of hospitalization in observation group were significantly less than those of control group （ P 〈 0.05 ）, morbidity of complications and mortality were significantly lower than those of control group （ P 〈 0.05 ）. The scores of quality of life at 3 months after treatment had been significantly raised （ P 〈 0.05 ）, and the degree of improvement in observation group was better than that of control group （ P 〈 0.05 ）. Conclusion The effect of isolated endovaseular surgery in treatment of thoracic aortic aneurysm is significant with shorter operating time, less blood loss, faster recovery of patients, and low rates of complications and mortality, thus it can raise the quality of life of patients with good long - term efficacy.
