特定企业妇女疾病谱变化的原因分析及对策

摘　　要：目的明确某石化企业已婚女职工常见妇科病的患病特点与变化趋势,制定有效的防治措施。方法采用流行病学回顾性研究方法,对某石化企业2002年3 357名与2012年3 122名已婚女职工接受妇女病普查情况进行对比分析。结果 2012年比2002年接受妇女病普查的人数显著减少,而妇女病患病率呈显著上升趋势。乳腺疾病和子宫肌瘤的发病率呈显著上升趋势（P〈0.01）;宫颈炎的发病率呈上升趋势（P〈0.05）;附件包块的发病率呈下降趋势（P〈0.05）;阴道炎的发病率无明显改变（P〉0.05）。31~50岁是妇女常见病的高发期,其中宫颈炎高发年龄段31~40岁,而乳腺疾病及子宫肌瘤高发年龄段41~50岁。结论妇女病总患病率居高不下,2个年度比较多数妇科疾病变化不大,乳腺疾病、子宫肌瘤、宫颈癌前病变的发病率升高,其中子宫肌瘤、宫颈癌前病变的发病率升高可能部分因为采用了液基细胞学检测。[ Objectives ] To determine the characteristics and the trend of common gynecological disease of mar- fled female workers in a petrochemical enterprise and to establish effective control measures. [ Methods ] A compar- ison and analysis were made between the 3 367 women in 2002 and 3 312 women in 2012 who were underwent Women＇s Disease Census in a petrochemical enterprise using retrospective epidemiological research methods. IRe- suits] The number of women in 2012 who underwent the Women＇s disease census is much smaller than that of in 2002, however, the prevalence of Women disease showed a significant upward trend; The incidence of breast disease and uterine fibroids showed a significant upward trend（P 〈0.01）, the incidence of cervieitis showed a upward trend （P 〈0.05）, the incidence of Adnexal mass decreased （P 〈0.05）, and the incidence of vaginitis has no significant change （P 〉0.05）. Women at the age between 31 and 50 years usually have a highly incidence of women disease, with cer- vicitis between 31 and 40 years, breast disease and uterine fibroids between 41 and 50 years. [Conclusions] The overall prevalence of disease remains high for women, there was no significant change of the majority women dis- eases between the two years＂ census. The morbidity of the breast disease, uterine fibroids, cervical lesions showed upward trend and the increase rate may be partly because the use of liquid-based cytology.
