摘  要:目的研究高危型人乳头瘤病毒(HPV)感染对年轻未婚女性宫颈病变的影响,病毒拷贝量与宫颈病变的关系。方法选取25岁以下年轻未婚女性758人进行HPV检测,对HPV检测阳性者,根据HPV拷贝量分3组,HPV在10^4拷贝组;HPV在10。~10。拷贝组;HPV在10^6拷贝以上组。结果共检测高危型HPV758人,阳性171人,阳性率2.3%。HPV检测在10^4拷贝以下26人;HPV在10^4~10^6拷贝116人;HPV在10^6拷贝以上29人。HPV在10^4拷贝以下时宫颈上皮细胞出现病变机会很低;当HPV在10^4~10^6拷贝时,宫颈上皮出现病变但转阴机会大;当HPV在10^6拷贝以上,发展为高度病变的可能性大;年龄在21岁以下未发现宫颈上皮内瘤变(CIN)2—3级病例。结论一个性活跃青少年HPV感染相当普遍,但多数属低度病变。Objective To study the influence of high -risk type human papilloma virus (HPV) infection on cervical lesions of young unmarried females, as well as the correlation between viral load and cervical lesion. Methods A total of 758 unmarried young women under 25 years old were selected for HPV detection, then the HPV positive cases were divided into 3 groups ac- cording to the HPV load( 10^4 copies group, 10^4 - 10^6 copies group and 〉 10^6 copies group). Results One hundred and seven- ty- one HPV positive cases were detected from the 758 young unmarried women, accounting for 2.3% (171/758), including 26 cases with HPV load below 10^4 copies, 116 cases between 10^4 ~ 10^6 copies and 29 cases above 10^6 copies. Cervical epitheli- al ceils lesions were infrequent in unmarried females with HPV load under 10^4 copies. When the HPV load was between 10^4 -10^6 copies, cervical epithelial lesions may appear, but with high proportion of positive -to- negative cases. Severe cervical le- sion was found frequent in females when the HPV load was above 10^6 copies. CIN2 - 3 cases were not found in women under 21 years old. Conclusion HPV infection is general in sexually active adolescents, and most were low- grade lesions.
