零缺陷理论在病案管理中的应用

摘　　要：目的追求和实现零缺陷质量目标,进一步提高病案管理质量。方法将零缺陷管理理论应用于病案管理中,通过全员教育和加强专业队伍建设,健全质量保证体系,实现质量标准化,消除影响质量管理的失控漏洞并实施持续质量改进,使病案管理工作逐步趋近零缺陷。结果病案回收工作准确率由实施零缺陷管理前的97.1%提高到100%;出院病案排序正确率由实施前的93.5%提高到99.8%;病案信息加工准确率由实施前的95.8%提高到99.2%;出院病案装订正确率由实施前的99.8%提高到100%;病案在架准确率由实施前的99.9%提高到100%,两年内未发生病案资料遗失或归档错误现象,病案服务的提供效率和准确率提高。各项质量管理措施得以落实,病案管理工作规范有序,进一步提高病案管理质量。结论零缺陷管理理论用于病案管理是切实可行的,零缺陷管理模式是促进病案管理先进性和科学性的有效方法。Objective To pursue and achieve zero defect quality, and to improve the quality of medical record management. Methods The ＂zero defect＂ management theory is applied to the medical record management. We make the management work of gradually toward zero defect through the full education and strengthening the construction of the professional team, improving quality assurance system, eliminating the influence of quality management control vulnerabilities and implementing continuous quality improvement. Results The accuracy of the medical records recovery is improved from 97.1% to 100% after the implementation of zero defect management; The accuracy of the medical records sorting increased from 93.5% to 99.8%; The medical record information processing accuracy rate increased from 95.8% to 99.2%; The medical records bookbinding accuracy increased from 99.8% to 100%; The accuracy of the medical record on the shelf increased from 99.9% to 100%. Within two years, the medical record lost or archiving mistake had never happened. The efficiency and accuracy of medical service are greatly increased. Various of quality management measures have been implemented. The medical records management is standardized and orderly. Conclusions The theory of Zero defect is appropriate in medical records management. Zero defect management mode is an effective way to promote the advanced and scientific of medical records management.
