尿血管紧张素原与IgA肾病患者新月体形成相关性分析

摘　　要：目的探讨IgA肾病(IgAN)患者尿血管紧张素原(angiotensinogen,AGT)与肾小球新月体形成的关系。方法入选北京大学第三医院肾内科2011年3月～2013年6月经肾脏活检病理检测证实的IgAN患者45例,ELISA检测尿AGT水平,计算新月体指数,分析二者的相关性。结果 IgAN患者尿AGT水平显著高于正常对照(212.3±276.1μg/g比6.6±4.9μg/g,P<0.05),有新月体患者显著高于无新月体患者(272.3±329.7μg/g比143.6±187.2μg/g,P<0.05),新月体指数≥10%的患者显著高于新月体指数<10%的患者(375.8±391.9μg/g比201.3±186.3μg/g,P<0.05),尿AGT与新月体指数(r=0.483,P=0.001)、24h尿蛋白(r=0.618,P=0.000)水平正相关,与血白蛋白(r=-0.535,P=0.000)水平负相关,与肾小球硬化程度无关。结论 IgAN患者尿液AGT水平可能反映肾组织新月体形成。Objective To analyze the relationship between urinary angiotensinogen （AGT） and the crescent formation in IgA nephropathy （IgA N） patients. Methods Forty-five patients diagnosed with IgA N by renal biopsy between March 2011 and June 2013 in Peking University Third Hospital were enrolled in this study. Enzyme linked immunosorbent assay （ELISA） was applied to detect urinary AGT, and crescent index was calculated. Correlation analysis between urinary AGT and crescent index was performed. Results Urinary AGT increased significantly in IgA N group as compared with that in control group （212.3±276.1 vs. 6.6±4.9 μg/g, P〈 0.05）. Urinary AGT was higher in IgA N patients with crescent formation than in those without crescent formation （272.3 ± 329.7 vs. 143.6 ±187.2 μg/g, P〈 0.05）. Urinary AGT was significantly higher in IgA N patients with crescent index≥10% than in those with the index 〈10% （375.8±391.9 vs. 201.3± 186.3 μg/g, P〈0.05）. Urinary AGT was positively correlated with crescent index （r=0.483, P=-0.001） and 24- h urine protein （r=0.618, P=0.000）, and negatively correlated with serum albumin （r=-0.535, P=0.000）. Urinary AGT was unrelated to renal glomerulosclerosis index. Conclusion Urinary AGT level may reflect the crescent formation in renal tissue in IgA N patients.
