摘　　要：目的探讨应用解剖在低压灌注下经尿道四区分割法等离子体前列腺剜切术的作用。方法患者取截石位,适当扩张尿道,耻骨上膀胱造瘘,实施膀胱内低压灌注模式,采用英国Gyrus等离子电切系统,置镜后通过观察尿道、精阜特点,应用解剖特点指导四区分割法前列腺剜切术,观察其疗效。结果 166例前列腺增生患者接受手术,通过电切镜术中观察和应用,了解其后尿道、精阜、前列腺尖的特点并总结出了"应用解剖特点",通过这些方法,患者术后国际前列腺症状评分(IPSS)、生命质量(QOL)、残余尿(RU)、最大尿流率(Qmax)得到明显改善(P<0.05)。结论掌握镜下后尿道及前列腺应用解剖特点,对实施前列腺剜切术有很大帮助,是保证手术效果的基础。Objective To investigate the clinical value of the anatomical features on the four-area division plasmakinetic enucleation of prostate under low-pressure perfusion. Methods Patients were in lithotomy position, and underwent the appropriate expansion of the urethra, suprapubic cystostomy, low-pressure perfusion patterns within the bladder, and observed the characterstics of urethra and verumontanum,using British Gyrus plasma electric cutting system. In order to research the clinical value of anatomical charactenstics on the removal of the prostate. Results 166 cases with benign prostatic hyperplasia underwent surgery. The anatomical features of the urethra, verumontanum and the prostatic apex through the intraoperative resectoscope and used those ＂ anatomical characteristics＂ to guide the four-fold division plasmakinetic enueleation of prostate. Conclusion Grasp the anatomical characteristics of the posterior urethra and prostate that can help the surgery of transurethral resection of prostate.
