应用两种不同导管预防导管相关感染的效果观察

摘　　要：目的比较两种不同导管在预防导管感染中的应用效果评价。方法选取医院麻醉科2010年1月-2013年1月100例患者,将其随机分为对照组和治疗组,每组50例,治疗组采用抗菌导管,对照组采用普通导管,采用生化检验方法鉴定抗菌导管和普通导管的病原菌生长状况。结果治疗组50例患者中肿瘤患者16例、非肿瘤患者34例,对照组50例患者中肿瘤患者13例、非肿瘤患者37例;治疗组有4例患者导管培养阳性,阳性率为8.0%,对照组有11例患者导管培养阳性,阳性率为22.0%,差异有统计学意义(P<0.05);治疗组中34例非肿瘤患者的导管培养均显示为阴性,对照组36例非肿瘤患者中6例导管培养阳性,阳性率为16.2%,差异有统计学意义(P<0.05)。结论导管相关性感染的发生率较高,患者的病死率、并发症发生率以及医疗费用增加,所以重视加强中心静脉插管的应用及管理,应用抗菌导管感染细菌阳性率低于普通导管,可以明显的降低感染率。OBJECTIVE To compare the effects of two types of catheters on prevention of deep venous catheter-related so as to achieve the high efficiency of application of catheters .METHODS From Jan 2010 to Jan 2013 ,a total of 100 patients who were treated in the department of anesthesia were enrolled in the study and randomly divided into the control group and the treatment group ,with 50 cases in each ,then the treatment group was adopted the antibacterial catheter ,the control group was treated with the common catheter ,and the growth of pathogens on the antibacterial catheter or the common catheter was observed with the use of biochemical assay . RESULTS Of the 50 patients in the treatment group ,there were 16 tumor patients and 34 patients without tumors . Of 50 patients in the control group ,there were 50 tumor patients and 37 patients without tumors .There were 4 （8 .0% ） patients in the treatment group with the catheters culture positive and 11 （22 .0% ） patients in the control group with the catheters cultured positive ,the difference was significant .The catheters were cultured negative for the 34 patients without tumors in the treatment group but were cultured positive for 6 （16 .2% ） of 36 patients without tumors in the control group ,the difference was significant （P〈0 .05） .CONCLUSIONS The incidence of central venous catheter-related infections is high , which increases the mortality rate and the incidence of complications as well as the medical cost .The positive rate of bacteria causing infections is lower in the patients who use antibacterial tubes than in those who use common tubes .It is necessary to pay high attention to the application of central venous catheters so as to significantly reduce the incidence of infections .
