摘 要:目的探讨评估增强磁共振成像(MRI)对脊椎术后感染的临床诊断价值,以提高感染诊断的准确性。方法选择2013年2月-2014年1月37例脊椎术后感染患者为研究对象,将其分为平扫组18例和增强组19例,平扫组患者行MIR平扫,而增强组患者行MIR增强扫描,观察比较两组患者的扫描影响病灶分析并比较两组诊断的准确性。结果平扫组椎间盘炎、脊椎骨髓炎、内膜外脓肿及软组织感染的显示率,分别为16.67%、11.11%、27.78%及22.22%,明显低于增强组各显示率36.84%、26.32%、15.79%、10.53%,差异有统计学意义(P<0.05);根据统计分析的结果,增强组的特异度、敏感度及准确度均明显优于平扫组,且差异有统计学意义(P<0.05)。结论增强MRI扫描可以有效提高影响信号的清晰准确性,改善MRI的敏感性、特异性及准确性,克服感染早期诊断困难的问题,具有极其重要的临床应用价值,是一种值得推广应用的脊椎术后感染诊断手段。OBJECTIVE To evaluate the clinical value of enhanced magnetic resonance imaging (MRI) in diagnosis of infections after spine surgery so as to improve the accuracy of diagnosis of infections .METHODS From Feb 2013 to Jan 2014 ,totally 37 patients with postoperative infections who underwent the spine surgery were enrolled in the study and divided into the unenhanced group with 18 cases and the enhanced group with 19 cases ,then the plain scan group received the plain MIR scan ,while the enhanced group received the enhanced MIR scan ,the effects of the scans on lesions were observed and compared between the two groups ,and the accuracy of diagnosis was compared between the two groups .RESULTS The display rate of discitis was 16 .67% in the plain scan group , significantly lower than 36 .84% in the enhanced group ;the display rate of spine osteomyelitis was 11 .11% in the plain scan group ,significantly lower than 26 .32% in the enhanced group ;the display rate of abscess outside the inner membrance was 27 .78% in the plain scan group ,significantly lower than 15 .79% in the enhanced group ;the display rate of soft tissue infections was 22 .22% in the plain scan group ,significantly lower than 10 .53% in the enhanced group ,the differences were significant (P〈0 .05) .The result of the statistical analysis showed that the specificity ,sensitivity ,and accuracy of the enhanced group were significantly better than those of the plain scan group (P〈0 .05) .CONCLUSIONS The enhanced MRI scan can effectively improve the accuracy ,sensitivity , and specificity of M RI and overcome the problems in early diagnosis of infections ;it has significant clinical value and is worthy to be promoted .
