复发性流产孕妇早期妊娠滋养细胞中E-钙黏素的表达与HPV感染的相关性

摘　　要：目的探讨E-钙黏素在复发性流产孕妇妊娠早期滋养层细胞中的表达与人乳头状瘤病毒(HPV)感染的相关性,以分析E-钙黏素在复发性流产中的作用。方法选取2012年1月-2013年6月妊娠早期复发性流产孕妇(研究组)与拟行人工流产术的早期妊娠妇女(对照组)各40例,取绒毛标本采用免疫组化法及PCR法分别检测绒毛滋养细胞中E-钙黏素的表达及HPV-DNA感染率,数据采用SPSS16.0统计软件进行分析。结果 E-钙黏素阳性率研究组为57.50%,对照组为92.50%,差异有统计学意义(P<0.05);HPV感染率研究组为35.0%,对照组为12.5%,HPV感染率研究组高于对照组,比较差异有统计学意义(P<0.05),研究组中HPV感染以HPV16为主占85.71%,对照组HPV16感染占40.00%,差异有统计学意义(P<0.05);HPV感染阳性的孕妇E-钙黏素阳性率研究组为35.71%,对照组为100.00%,差异有统计学意义(P<0.05)。结论在妊娠早期滋养细胞中存在E-钙黏素的阳性表达,E-钙黏素表达下调可能与复发性流产相关,推测HPV感染,特别是HPV16感染可能是引起E-钙黏素的异常表达的因素之一。OBJECTIVE To evaluate relationship between the expression of E-cadherin in early phase gestational trophoblastic cells to HPV infection for recurrent abortion patients , so as to analyze E-calcium in recurrent abortion .METHODS During Jan .2012 to Jun .2013 ,40 pregnant women with recurrent abortion in first trimester （research group） and fitting line of induced abortion of first trimester 40 women （control group） were collected , obtained specimens of villi , and using immunohistochemical method and PCR method villi nourish cells was detected in the expression of E-calcium stick , and HPV DNA . SPSS16 .0 software was used for statistical analysis .RESULTS The 57 .50% in research group and 92 .50% in control group . The degree of positive expression in research group was lower than that in control group with significant difference between the two groups （P〈0 .05） .The research group HPV infection rate was 35 .00% ,and the control group infection rate was 12 .50% .HPV infection of the research group was higher than control group （P〈0 .05） .The research group of HPV infection was given priority to HPV16 ,accounted for 85 .71% of infections ,HPV16 infection rate was 40 .00% in the control group .HPV16 the research group was significantly higher than the control group （P〈0 .05） .The pregnant women in research group with HPV infection got positive E-calcium accounted for 35 .71% , the control group was 100 .00% ,and the positive expression rate in research group was lower than the control group （P〈0 .05） .CONCLUSIONS In the early weeks of pregnancy nourish cells exist the positive expression of E-cadherin .E-cadherin express′s reducing may be associated with recurrent miscarriage ,and speculated that HPV infection may be one of the reasons to cause the abnormal expression of the E-calcium stick .
