人乳头状瘤病毒感染对宫颈上皮内瘤变宫颈环形电切术预后的相关性分析

摘　　要：目的分析高危型人乳头状瘤病毒(HR-HPV)感染的检测对宫颈上皮内瘤变(CIN)宫颈环形电切术(LEEP)预后的相关性,探讨其对宫颈癌发病的判断作用,以指导早期干预,改善患者预后。方法选取医院2009年5月-2010年5月收治的165例HR-HPV感染且行LEEP治疗的CIN患者,按照其复发或残留分为复发残留组71例及无复发残留组94例,进行分组对比,数据采用SPSS13.0进行分析。结果 165例患者中有71例出现复发、残留,发生率43.0%;复发残留组切缘阴性/HR-HPV阳性及切缘阳性/HR-HPV阳性例数明显高于无复发残留组;Fisher精确检验显示,HR-HPV与CIN预后存在显著相关性。结论 HR-HPV负荷量的上升表示CIN患者预后不良,因此,对HR-HPV的早期筛查和监测,是降低患者复发、残留率及宫颈癌发病率的关键。OBJECTIVE To analyze the correlation between the high-risk human papillomavirus （HR-HPV ） infection detection and prognosis of the loop electrosurgical excision procedure （LEEP） for treatment of cervical intraepithelial neoplasia （CIN） and explore its role in judgment of cervical cancer ,so as to provide guidance for early intervention and improve the prognosis of patients .METHODS A total of 165 CIN patients infected with HR-HPV and treated by LEEP ,treated in the hospital from May 2009 to May 2010 ,were divided into the recurrence or residual group （71 cases） and non recurrence or residual group （94 cases） according to their recurrence or residual for comparison ,and SPSS13 .0 was adopted for statistic analysis .RESULTS Of 165 patients ,71 cases suffered recurrence or residual ,with an incidence rate of 43% ;negative margin/HR-HPV positive and positive margin/HR-HPV positive cases of the recurrence or residual group were significantly higher than those of the non recurrence or residual group ;Fisher exact test showed there was a significant correlation between HR-HPV and CIN prognosis .CONCLUSIONS The rise of HR-HPV load indicates poor prognosis of patients with CIN , therefore ,early screening and monitoring of HR-HPV are critical to reducing recurrence ,residual rates as well as cervical cancer incidence rates of the patients .
