产前超声诊断胎儿双主动脉弓畸形的临床价值

摘　　要：目的探讨产前胎儿超声心动图对诊断胎儿双主动脉弓畸形的价值。方法分析产前超声诊断为双主动脉弓36例胎儿超声图像特征,追踪其临床结局。结果 36例双主动脉弓胎儿均通过超声三血管-气管切面作出诊断。其中,单纯双主动脉弓24例,合并心内畸形9例,合并心外畸形3例。有随访结果的14例单纯双主动脉弓、5例合并心内畸形和3例合并心外畸形者均得到影像学或尸检证实;仅2例合并心内畸形胎儿的初始诊断与结果不符。结论胎儿双主动脉弓有特征性超声表现,通过三血管-气管切面可以在胎儿时期有效检出双主动脉弓畸形,对评估胎儿预后及指导后期治疗有重要临床价值。Objective To investigate the clinical value of prenatal ultrasonography in diagnosis of double aortic arch malformation in fetus. Methods Echocardiographic features of fetal double aortic arch in 36 cases detected by prenatal ultrasonography were analyzed and the diagnosis was followed up. Results Thirty-six fetuses were diagnosed as double aortic arch malformation by three-vessel trachea view, of whom 24 cases were with isolated double aortic arch, 9 cases with complicated with cardiac abnormalities, and 3 cases with complicated with extracardiac abnormalities. Of the fetuses with complete follow-up data, 14 cases of isolated double aortic arch, 5 cases of complicated with cardiac abnormalities, and 3 cases of complicated with extracardiac abnormalities were confirmed by postnatal echocardiography or combined CT examination. Two cases of complicated with cardiac abnormalities did not be confirmed after birth. Conclusion Fetal double aortic arch malformation has the characteristic echocardiographic appearances. Prenatal echocardiography by three-vessel trachea view can effectively show the double aortic arch malformation in the fetus and has an important clinical value in making diagnosis, predicting the pregnoss and guiding treatment of fetal double aortic arch malformation.
