摘  要:目的探讨联合检测甲状腺激素及甲状腺过氧化物酶在产前筛查中的意义。方法 805例孕妇按孕期分为孕早期(A组,288例)、孕中期(B组,255例)和孕晚期组(C组,262例)三组,采用化学发光法检测促甲状腺激素(TSH)、游离甲状腺三碘原氨酸(FT3)、游离甲状腺素(FT4)、甲状腺三碘原氨酸(T3)、甲状腺素(T4)和甲状腺过氧化物酶抗体(TPOAb)水平。结果 C组血清TSH水平高于B组和A组(P<0.05);A组血清FT3、FT4、T3和T4水平高于B组和C组(P<0.05)。TPOAb阳性组孕妇TSH异常的发生率高于阴性组(P<0.05)。结论孕妇血清TSH、FT3、FT4、T3和T4水平在不同孕期存在明显差异,可作为孕期筛查的指标。TPOAb阳性的孕妇更容易发生甲状腺功能紊乱,也应作为筛查指标之一。Objective To explore the significance of combined detection of serum thyroid hormones and thyroid peroxidase in prenatal screening. Methods A total of 805 pregnant women in different pregnancy stages was divided into three different groups of A(in the first trimester, 288 cases),B(in the second trimester, 255 cases) and C(in the third trimester, 262 cases). Serum levels of thyroid stimulating hormone ( TSH ), free triiodothyronine ( FT3 ), free thyroxine ( FT4 ), triiodothyronine (T3), thyroxine (T4) and thyroid peroxidase antibody were detected by chemiluminescence immunoassay. Results Serum TSH was higher in group C than that in groups of B and A(P〈0.05). Serum levels of FT3,FT4,T3 and T4 were higher in group A than those in groups of B and C(P〈0.05). The incidence rate of abnormal TSH was higher in the pregnant women with TPOAb positive than those with TSH negative(P〈0.05). Conclusion Serum levels of TSH, FT3, FT4, T3 and T4 are different in the pregnant women in different pregnancy trimesters, which may be considered as the indicators during screening. TPOAb positive pregnant women are more prone to having thyroid dysfunction and it should be taken as an indicator during screening.
