摘 要:目的:研究大鼠肠道术后不同时间段的疲劳程度和中枢海马区5-羟色胺(5-HT)、外周炎性因子的改变,并探讨亮氨酸(Leu)对术后疲劳的改善作用。方法:将32只大鼠行颅内置管,并随机分为正常组(N组)、假手术组(S组,开关手术)、模型组(M组,70%小肠切除术)和Leu组(C组,在M组基础上用Leu干预)。术后当天及第1、3、5、7天分别观察其行为变化;用微透析法收集大鼠海马组织间隙的透析液,再用高效液相荧光检测法检测其中5-HT浓度;ELISA法检测外周血清白细胞介素(IL)-1β和IL-6。结果:S、M、C三组大鼠中枢海马区5-HT的变化趋势相似,手术当天升至最高,第1天回落,第3天再次升高,第5、7天逐渐降至正常水平。术后第3、5、7天M组海马内5-HT均较C组高,差异均有统计学意义(P<0.05)。除N组外,IL-6在第1、3、5、7天组间均有显著性差异(P<0.05),第3天达高峰,尔后逐渐下降。但外周血清IL-1β差异均无统计学意义(P>0.05)。旷场实验显示,C组行为学上较M组有明显改善。结论:手术应激可影响大鼠中枢海马5-HT。Leu可改变术后脑内5-HT的变化,并能显著改善大鼠肠道手术后的疲劳程度,减轻炎性反应。Objective:To investigate the changes of 5-HT in hippocampus and peripheral inflammatory factors during different periods after intestinal surgery in rats,and to search the leucine (Leu) 's anti-fatigue effects.Methods:32 male rats were randomly divided into four assessment groups,including normal group (Group N),sham-operated Group (Group S),model group (Group M,after 70% enterectomy) and Leucine-treated Group (Group C).Postoperative behaviors of rats were assessed after surgery.Dialysates from hippocampus tissues were collected,and measured by HPLC.Serum interluekin (IL)-1β and IL-6 were measured by ELISA kits.All the processes were taken at the day 0,1,3,5,7 after the intestinal surgery.Results:Except for Group N,levels of 5-HT in the other three groups had a similar trend.On the day of surgery,5-HT was risen to the peak,but reduced rapidly to the bottom at postoperative day 1.And it elevated again at postoperative day 3,and then gradually recovered to the preoperative level.The concentrations of 5-HT in Group M after surgery were higher than those in Group C at the same time respectively (P 〈 0.05).For IL-6,beside Group N,there were significant differences in each group at the day 1,3,5 and 7 after surgery (P 〈 0.05).It climbed to the summit at the day 3,and gradually fall back to the preoperative level.At each day,the IL-6 level in Group C were lower than those in Group M respectively (P 〈 0.05).However,for IL-1β,no significant differences were found between these four groups.Behaviorally,open field test illustrated that Group C was better than Group M.Conclusion:Surgical stress to rats may affect the variations of 5-HT in hippocampus,make the trend of these changes up-down.Otherwise,Leu can effectively alleviate postoperative fatigue,and reduce the inflammatory reaction.
