摘  要:目的探讨相同剂量(声功率×辐照时间)的高强度聚焦超声(HIFU)所致离体牛眼晶状体凝固性坏死的时变规律。方法用等剂量HIFU(100W×6s、200W×3s、300W×2s、600W×1s)辐照离体牛眼晶状体,每组重复20次,用高速摄像系统记录凝固性坏死的时变情况,每隔一定时间测量凝固性坏死的长轴和短轴,计算体积并分析时变规律。结果 100W×6s、200W×3s、300W×2s,600W×1s辐照后开始出现凝固性坏死的平均时间分别为1.90s、0.75s、0.44s、0.36s;100W辐照直至停止(6s)时,凝固性坏死体积呈线性增长,增长速率为6.20mm3/s;200W、300W辐照的增长趋势类似,分别辐照至2.3s、1.5s处增长趋势由线性变为加速增长至辐照结束,且线性部分增长速率分别为13.49mm3/s和25.21mm3/s;600W辐照直到停止(1s)过程中凝固性坏死呈非线性增长,四组参数辐照所致凝固性坏死的长宽比均随辐照时间呈下降趋势。结论随着声功率增加,出现凝固性坏死所需辐照时间缩短,继续辐照后凝固性坏死体积由线性增长变为非线性快速增长趋势的时间缩短,且线性增长部分速率增加。总剂量相同声功率越高,HIFU非线性效应越明显。Objective To investigate the time-varying regularity of coagulation necrosis induced by high-intensity focused ultrasound (HIFU) with same dose by high-speed photography. Methods HIFU of same dose (600 J, 100 W×6 s, 200 W × 3 s, 300 W×2 s, 600 W × 1 s) was used to irradiate bovine eye lens in vitro, and each group repeated 20 times. The real-time formation of coagulation necrosis was recorded using high speed imaging device, and then the volume at same interval within each group was calculated. Results The respective time when coagulation necrosis first formed in group 100 W×6 s, 200 W×3 s, 300 W×2 s, 600 W×1 s was 1.90 s, 0.75 s, 0. 44 s, 0.36 s, respectively, then the lesions grew linearly (exposed to 100 W) until exposure stopped. When exposed to 200 W×3 s, 300 W×2 s, the necrosis grew slowly for a short time when first formed, and then grew rapidly at the point of 2.3 s, 1.5 s, and the according growth rate was 13.49 mm^3/s and 25.21 mm^3/s, respectively. Group 600 W × 1 s induced coagulation necrosis developed nonlinearly till exposure stopped. The ratio of longitudinal axis to short axis reduced with time in all groups. Conclusion With increase of sound power, the time required to induce visible lesions in bovine eye lens shortens, the time when growth tendency turns to rapid nonlinear from linearly shortens accordingly, and the growth rate in linear stage increases, too. Besides, the nonlinear effect of same dose HIFU turns evident.
