跟骨复位器配合克氏针微创治疗跟骨关节内移位骨折30例  

Clinical Observation On the Calcaneal Reductor With K-wire in Minimally Invasive Treatment of Displaced Intra-articular calcaneal fracture

摘　　要：目的:探讨跟骨复位器配合克氏针撬拨治疗跟骨关节内移位骨折的临床疗效。方法:自2011年2月-2013年6月对苏州市中医医院30例36足跟骨关节内骨折,采用跟骨复位器配合克氏针撬拨治疗。其中男性26例,女性4例;按照Sanders分型标准:SandersⅡ型21足,SandersⅢ型15足。术前及术后测Bhlers角和Gissanes角,足部功能按Maryland足部评分系统评分。结果:30例患者全部获得随访,足部功能按照Maryland评分系统评分:SandersⅡ型优良率95.2%,SandersⅢ型优良率86.6%,术后3月测量SandersⅡ型Bhlers角为(33.2±2.9)°,Gissanes角(137.5±2.6)°与术前比较差异均有统计学意义(P<0.01);SandersⅢ型Bhlers角为(33.2±2.9)°,Gissanes角(137.5±2.6)°,与术前比较差异均有统计学意义(P<0.01)。结论:采用跟骨复位器配合克氏针撬拨治疗跟骨关节内移位骨折具有明显的临床特色和临床疗效,值得推广。Objective：To discuss the clinical efficacy in the treatment of displaced intra-articular calcaneal fracture by calcaneal reductor with Kirschner wire poking. Methods： From February 2011 to June 2013 in Suzhou City Hospital of traditional Chinese medicine there were 30 cases of 36 feet of intra-articular calcaneal fractures treated with calcaneal reductor combined with Kirschner wire poking , of which 26 males, 4 females; according to the Sanders classification： there were Sanders type 21, Sanders Ⅲ 15 feet. Preoperative and postoperative B ？ hlers angle and Gissanes were measured, meanwhile the foot function evaluated according to the Maryland foot score system. Results： All 30 patients were followed up, foot function scored according to the Maryland scoring system , the excellent and good rate was 95.2% in Sanders Ⅱ , the excellent and good rate was 86.6% in Sanders Ⅲ, 3 months after the operation in Sanders Ⅱ B hlers angle was （33.2± 2.9） °, Gissanes angle （137.5 ± 2. 6） °,compared with those before operation there were statistically significance （P〈 0.01）; while in Sanders type Ⅲ B hlers angle was （33.2 ±2.9） °, Gissanes angle was （137.5 ±2.6） °, compared with those before operation there were statistically significant （P〈 0. 01）. Conclusion：There are obvious clinical features and curative effect in treatment of displaced intraarticular calcaneal fractures by calcaneal reductor with Kirschner wire poking, so it is worth promoting.
