小腿穿支皮瓣修复胫前深度烧伤创面疗效观察

摘　　要：目的探讨小腿穿支皮瓣修复胫前深度烧伤创面的方法及疗效。方法采用小腿内侧、前外侧、后外侧及后侧面积为9 cm×3 cm^16 cm×12 cm的穿支皮瓣修复50例面积为7 cm×2 cm^13 cm×10 cm的胫前深度烧伤创面,观察治疗效果。结果除3例患者植皮后创面出现皮瓣尖端坏死外,其余皮瓣均成活良好;术后随访6个月~1年,所有患肢均无功能障碍,皮瓣外观和质地良好。结论小腿穿支皮瓣血供可靠,手术操作简便,术后创面外形、功能恢复良好,是胫前深度烧伤创面安全、有效的修复方法。Objective To explore the method and curative efficacy of shank perforator flap in treatment of deep pretibial burn wounds. Method Shank perforator flaps of 9 cm × 3 cm^16 cm × 12 cm size on inner side, front outer side, back outer side and back side were adopted to repair 50 cases of deep pretibial burn wounds measured 7 cm × 2 cm^13 cm × 10 cm. The treatment effects were observed. Result All flaps well survived after skin grafting, except that top flap necrotized in three patients. In follow-up visits six months to one year after surgery all diseased limbs were free from disfunc-tion and the appearance and quality of flap were good. Conclusion Shank perforator flaps enjoy reliable blood supply and convenient operation with good postoperative restoration of appearance and function of wounds, being an effective and safe means of repairing pretibial deep wounds.
