不同剂量的盐酸帕罗诺司琼预防眼球摘除加义眼植入术后恶心呕吐(PONV)的效果

摘　　要：目的 观察3种不同剂量的盐酸帕罗诺司琼预防眼球摘除加义眼植入术后恶心呕吐（postoperative nausea and vomiting,PONV）的效果及其安全性。方法 择期全麻下行眼球摘除加义眼植入手术的患者75名,ASA Ⅰ或Ⅱ级,年龄18-70岁,随机分为3组：A组,帕洛诺司琼2.5 μg/kg （n=25）;B组,帕洛诺司琼5 μg/kg （n=25）;C组,帕洛诺司琼7.5 μg/kg （n=25）。均予麻醉诱导前静注不同剂量的帕洛诺司琼,观察术后0-4 h、4-24 h、24-48 h和48-72 h的PONV严重程度。结果 3组患者24-48 h恶心评分的差异有统计学意义（P〈0.05）,C组的评分均显著低于A组和B组（P〈0.05）,A、B两组之间的差异无统计学意义（P〉0.05）,其余时段各组间的差异均无统计学意义（P〉0.05）。3组呕吐发生率均低于32%,而各时段呕吐评分的差异均无统计学意义（P〉0.05）。结论 7.5 μg/kg的盐酸帕洛诺司琼预防眼球摘除加义眼植入PONV效果最佳。Objective To observe the efficacy and safety of three different doses of palonosetron on the incidence and severity of postoperative nausea and vomiting （PONV） for eye enucleation and orbital hydroxyapatite implant surgery.Methods Seventy-five patients of ASA Ⅰ or Ⅱ aged between 18 and 70 years old ,and scheduled for eye enucleation and orbital hydroxyapatite implant surgery.They were assigned into 3 groups randomly：Group A （n=25）,palonosetron 2.5 μg/kg; Group B （n=25）,palonosetron 5 μg/kg; Group C （n=25）,palonosetron 7.5 μg/kg.All patients who received different doses of palonosetron immediately before induction of anaesthesia.The severity of nausea and vomiting were evaluated during the 0 to 4h,4 to 24 h,24 to 48h and 48 to 72 h postoperative time interval.Results There was significantly difference among 3 groups of nausea severity during the intervals of 24-48 h （P〈0.05）.Scores of group C were strictly lower than group A and B （P〈0.05）,but no significantly difference were found between group A and B.The vomiting incidences of all groups were all under 32%,and vomiting severity among all groups were similar during all intervals.Conclusions The dose of palonosetron at 7.5 μg/kg maybe best to prevent PONV for eye enucleation and orbital hydroxyapatite implant surgery.
