摘  要:目的应用超声心动图评价急性ST段抬高型心肌梗死(STEMI)患者急诊经皮冠状动脉内介入术(PCI)2个月内的舒张功能,探讨急诊PCI术对STEMI患者短期内左室舒张功能的影响。方法对入选的39例STEMI患者分别于急诊PCI术前、术后即刻及术后2周、2个月行超声心动图检查。检测指标:二尖瓣口舒张期血流E峰、A峰、E/A;左房容积指数(LAVI);二尖瓣环4个位点的舒张早期运动速度E、计算E/E。结果(1)STEMI患者PCI术后侧壁E较术前减低(P<0.05);(2)左房容积指数(LAVI)在术后2周、2个月较术前及术后即刻明显增大(P<0.05);(3)后间隔E在4个时间点的变化趋势无显著差异(P>0.05),但均<8 cm/s。结论 STEMI患者急诊PCI术后2周、2个月左室舒张功能障碍持续存在。Objective The objective of this study was to investigate the short-term effect of emergency percutaneous coronary intervention (PCI) surgery on the diastolic function of left ventricular in patients with acute ST segment elevation myocardial infarction (STEMI).Methods Echocardiogram examinations were conducted on 39 cases of STEMI patients at elaborately designed times,i.e.,prior to,immediately after,two weeks and two months after the PCI surgery,respectively.Measured parameters:diastolic mitral inflow velocity E peak,A peak and E/A ratio,left atrial volume index (LAVI),early diastole velocity E' at four mitral valve annulus sites,and extracted E/E' ratio.Results Compared correspondingly to what were measured prior to the surgery,(1) the E values at posterior wall decreased withP〈0.05,respectively.(2) The left atrial volume index (LAVI) increased significantly in two weeks and two months after the surgery,respectively(P〈0.05).(3) All E velocities were smaller than 8 cm/s,but the change trends of E velocities (at the posterior wall) observed at different four times were no significant.Conclusions Two weeks and even two months after the PCI surgery,left ventricular diastolic dysfunction persists among STEMI patients.
