局灶性脾结核的超声造影表现

摘　　要：目的探讨脾结核的超声造影声像图特征。方法分析32例经穿刺病理活检证实为脾结核患者的超声造影表现,总结脾结核的超声造影特征。结果根据超声造影表现分为环状增强、分隔样增强及无增强3种类型。I型:环状增强型(17例),表现为病灶周边见造影剂填充,呈环状增强,内部无造影剂填充,呈无增强;Ⅱ型:分隔样增强型(4例),表现为病灶边缘性增强,内部见纤细条索状分隔样增强,其余部分呈无增强;Ⅲ型:无增强型(11例),表现为病灶无造影剂填充,呈无增强。结论超声造影对脾结核的血流灌注可提供有价值的信息,有助于脾结核的诊断。Objective To explore the characteristics of contrast-enhanced ultrasound（CEUS） in the diagnosis of splenic tuberculosis.Methods A total of 32 cases clinically diagnosed by biopsy for splenic tuberculosis were examined by CEUS,the performance of CEUS was summarized.Results The CEUS results were divided into three types：annular enhancement,separated enhancement and non-enhancement.17 cases of the first type displayed annular enhancement of the mass and the inside region with no enhancement.4 cases of the second type displayed margin enhancement and inside region with slight separated enhancement.11 cases of the third type showed no enhangce of the mass.Conclusions CEUS can provide useful informations of the blood perfusion in splenic tuberculosis and may contribute to the diagnosis.
