胰腺实性假乳头状瘤的超声造影表现

摘　　要：目的分析胰腺实性假乳头状瘤(SPTP)的二维声像图特征及超声造影表现,探讨超声造影对SPTP的诊断价值。方法回顾性分析16例经手术病理证实的SPTP患者的二维及超声造影表现。结果 SPTP多见于青年女性,好发于胰头部,边界清晰,多数有包膜,呈囊实混合或实性肿块,少数可见较粗大钙化。超声造影显示11例肿瘤与胰腺同步或稍晚于胰腺增强,瘤内造影剂退出早于胰腺,强化程度等于或弱于胰腺;2例肿瘤实质早于胰腺增强伴无增强区,强化程度强于胰腺,造影剂退出同于胰腺;2例肿瘤实质与胰腺实质同步增强,造影剂未见明显退出;1例肿瘤内部无明显增强,病理示肿瘤组织绝大部分坏死;部分肿瘤可见包膜环状等增强及分隔样强化。结论 SPTP的超声造影表现具有一定特征性,可对诊断提供重要信息,有较高的临床诊断价值。Objective Analyze the imaging features of solid pseudopapillary tumor of the pancreas （SPTP） with two-dimensional ultrasound（US） and contrast-enhanced ultrasound（CEUS）,explore the value of CEUS in the diagnosis of SPTP.Methods The 2D ultrasound and CEUS images of sixteen patients with SPTP confirmed by pathology were retrospectively analyzed.Results Most SPTP can be found in young females.Tumors located more at head of pancreas.SPTP appeared as a well-defined lesion with capsule,while cystic-solid masses or solid masses were showed,and macro-calcification was detected in some patients on US.On CEUS,eleven tumors enhanced simultaneously or slightly late compared with normal pancreatic tissues.The contrast agent washed out quickly in all tumors than in normal pancreatic tissues.The enhanced degrees were equal to or less than that of the normal pancreatic tissues;two cases of tumor parenchyma enhanced earlier than pancreatic tissues with non-enhancement area,the enhanced degrees were more than that of pancreatic tissues,contrast agents washed out the same as the pancreas tissues;another two cases of tumor parenchyma enhanced same as pancreatic tissues,contrast media did not obviously exit; the rest one appeared as non-enhancement inside in the arterial phase with most tumor tissue necrosis showed on pathological examination.Some tumors showed capsule and septum enhancement.Conclusions The manifestation of SPTP on CEUS has some features and can provide important information for diagnosis,has high value of clinical diagnosis.
