电热砭与传统艾灸治疗寒湿型腰痛:随机对照研究

摘　　要：目的:比较电热砭石与艾盒灸两种方法在治疗寒湿型腰痛方面的疗效差异。方法:将41例寒湿型腰痛患者随机分为电热砭石组(26例)和艾盒灸组(15例)。电热砭石组将电热砭石置于肾俞、委中穴,调节电热砭石的温度,以患者舒适为度;艾盒灸组于肾俞、委中穴行艾盒灸治疗,均每日或隔日1次,10次为一疗程。观察患者治疗前后腰痛症状体征积分、寒湿证候积分的变化情况。结果:两组均可显著改善患者腰痛症状体征积分与寒湿证候积分(均P<0.01);电热砭石组症状体征变化率和证候变化率分别为(37.04±32.68)%、(22.85±29.95)%,艾盒灸组分别为(47.29±22.08)%、(23.89±22.53)%,组间差异无统计学意义(均P>0.05)。结论:电热砭石治疗寒湿型腰痛疗效与艾盒灸相似,其操作更为方便、安全,环境污染小。Objective To compare the difference in the efficacy on low back pain of cold-damp pattern between electric-thermal Bian-stone therapy and moxibustion box therapy. Methods Forty-one cases of low hack pain of cold-damp pattern were randomized into an electric-thermal Elan-stone therapy group （group A, 26 cases） and a box moxibustion therapy group （group B, 15 cases）. In the group A, the electric-thermal Bian-stone was placed over Shenshu （BL 23） and Weizhong （BL 40）. The temperature of stone was adjusted in accordance with patient＇s comfort. In the group B, moxibustion box was used over Shenshu （BL 23） and Weizhong （BL 40）. The treatment was given once every day or every two days. Ten treatments made one session. The symptom and physical signs score of low back pain and the score of cold-damp syndrome were observed before and after treatment in the pa- tients. Results The symptom and physical signs score of low back pain and the score of cold-damp syndrome were all improved in the two groups （all P〈0.01）. The changing rate of symptom and physical signs and syndrome were （37.04±32.68）% and （22.85±29.95）% in the group A, and were （47.29±22.08）% and （23.89±22.53） % in the group B, respectively, without significant difference in comparison between the two groups （all P〈0. 05）. Conclusion The effieacy on low back pain of cold-clamp pattern treated with the electric-thermal Bianstone therapy is similar to that of moxibuation box therapy. This therapy is characterized as more convenient, safer operation and less pollution.
