摘  要:目的探讨血Clara细胞分泌蛋白(CC16)及肺表面活性蛋白A(SP-A)在新生儿急性肺损伤(ALI)与急性呼吸窘迫综合征(ARDS)中的诊断意义。方法选取某院收治的ALI患儿16例作为ALI组,选取同期收治的ARDS患儿7例作为ARDS组,并选取同期健康新生儿25例作为对照组,采取酶联免疫吸附法(ELISA)对3组新生儿进行血清CC16及SP-A水平检测,比较3组血清CC16及SP-A水平。结果 ALI组及ARDS组血清CC16及SP-A水平均低于对照组,差异有统计学意义(P<0.05)。ALI组血清CC16及SP-A水平均低于ARDS组,差异有统计学意义(P<0.05)。结论血清CC16及SP-A的检测运用于新生儿ALI与ARDS的诊断中较为敏感,并且可作为ALI及ARDS的鉴别指标使用。Objective To investigate the blood Clara cell secretory protein (CC16) and pulmonary surfactant protein A (SP-A) in the diagnosis of neonatal acute lung injury (ALI) and acute respiratory distress syndrome (ARDS). Method 16 cases of newborns with ALI received and treated in the hospital were selected for ALI group and 7 cases of newborns with ARDS received in the same period were selected as ARDS group and 25 cases of healthy newborns were in the control group. Enzyme-linked immunosorbent assay (ELISA) was used to detect serum CC16 and SP-A levels in the three groups. Results Serum CC16 and SP-A levels of ALI and ARDS groups were lower than the control group and the difference was statistically significant (P〈0.05). Serum CC16 and SP-A levels of ALI group were lower than ARDS group and the difference was statistically significant (P〈0.05). Conclusion Serum CC16 and SP-A are sensitive in the diagnosis of neonatal ALI and ARDS and can be used as identification index of ALI and ARDS.
