经脐单通道常规器械腹腔镜胆囊切除术56例临床观察

摘　　要：目的探讨经脐单通道常规器械腹腔镜胆囊切除术的临床应用价值。方法回顾性分析重庆市北碚区中医院2012年1月-2013年1月56例经脐单通道常规器械腹腔镜胆囊切除术患者的临床资料。结果47例成功施行经脐单通道腹腔镜胆囊切除术，6例因术中暴露困难、出血不易处理而改为传统的3孔腹腔镜手术，3例因胆囊周围炎症较重而改为开腹胆囊切除手术。术后患者恢复良好，无并发症发生，2～4d出院。随访1个月脐部无明显瘢痕。结论经脐单通道常规器械腹腔镜胆囊切除术安全可行，具有创伤小、美容效果好、术后恢复快等优点，是现阶段有技术条件的医院值得推广的无疤痕技术。Objective To probe into the clinical application value of transumbilical single-channel endoscopic cholecystectomy performed using conventional instruments. Method Clinical data of 56 cases of patients undergone transumbilical single-channel endoscopic cholecystectomy performed using conventional instruments in Chongqing Beibei District Hospital of Traditional Chinese Medicine from January 2012 to January 2013 was retrospectively analyzed. Results 47 cases had transumbilical single-channel endoscopic cholecystectomy successfully, 6 cases were changed to traditional three-port laparoscopic operation because of exposure difficulty and that bleeding was not easy to handle during the operation, and 3 cases were changed to open cholecystectomy due to serious pericholecystitis. Patients recovered well after operation and there were not any complications. They were discharged from the hospital after 2~4 d, and there were no obvious scars in the umbilicus found through a one-month follow up. Conclusion Transumbilical single-channel endoscopic cholecystectomy performed using conventional instruments is safe and operable with advantages of small wound, good cosmetic effect and fast postoperative recovery, which is a scarless technology worth promotion in hospitals satisfying corresponding technical conditions at the present stage.
