脂肪来源基质细胞对超长皮瓣成活比例的影响

摘　　要：目的探讨脂肪来源基质细胞对超长皮瓣成活的影响。方法利用胶原酶消化sD大鼠腹股沟区的脂肪，经过分离培养传代，将获得的大量脂肪基质细胞行Dil染色，并在sD大鼠背部形成超比例任意皮瓣模型；将Dil染色后的脂肪基质细胞注射到皮瓣下治疗，并与生理盐水治疗组作对比；观察皮瓣大体情况，计算皮瓣成活率；观察病理切片毛细血管密度，荧光显微镜检测脂肪基质细胞在皮瓣内的分布和存活状况。结果术后第10天，皮瓣成活与坏死部分界线清楚，脂肪基质细胞组皮瓣成活率明显高于生理盐水组。脂肪基质细胞组和生理盐水组的皮瓣存活率分别为（67．4±5．1）％和（52．5±6．5）％，P〈0．05；毛细血管密度分别为（57．1±4．7）／mm。和（28．7±2．8）／mm2，P〈0．05；脂肪基质细胞注射入SD大鼠皮瓣后，脂肪基质细胞存活并参与新生血管的形成。结论脂肪基质细胞可明显增加超比例任意皮瓣成活面积，其存临床治疗中有潜在的应用和研究价值。Objective To explore the influence of adipose-derived stromal cells on over-long flap surviv- al. Methods The collagenase was used to digest fat from groin area of SD rat, the stromal cells obtained from separation culture and passages were stained by DiI. A over-proportionate random flap was formed at back of SD rat model, the stained adipose derived stromal cells were injected into the skin flap for treatment, physiological saline was also injected as control. General situation of the flap was observed, flap survival rate was calculated, the capillary density was observed by pathological section and the distribution of adipose-derived stromal cells within the skin flap and survival state were detected by fluorescence microscope. Results At 10 days after op- eration, boundaries of the survival flap and the necrotic part was clear, The flap survival rate in the adipose-de- rived stromal cells group was obviously higher than that in the control group, they were （67.4 ± 5.1 ） % and （52.5 ± 6.5 ） % respectively （ P〈 0.05 ）. The blood capillary density in two groups were （57.1 ± 4.7）/mm2 and （28.7 ± 2.8 ）/mm2, respectively （ P 〈 0.05）. After injection of adipose stem cells into the SD rat skin flap, adipose-derived stromal cells survived and participated in the angiogenesis. Conclusion Adipose-derived stromal cells can significantly increase the survival area of the over-proportion random flap, it has potential worth of application and research in the clinical treatments.
