国内近5年肺孢子菌肺炎4153例分析

摘　　要：目的分析国内近5年肺孢子菌肺炎(pneumocystis pneumonia PCP)发病与诊疗状况,从而为PCP诊治提供参考。方法检索国内2008—2012年发表的有关PCP论文,并对文献中PCP的病例数、基础病、诊疗等情况进行分析。结果①共检索到PCP相关的文献报道298篇;②PCP患者共4153例,其中3518例(84.7%)PCP与HIV/AIDS相关,285例(6.9%)PCP患者与器官移植相关,恶性肿瘤相关PCP患者94例(2.3%),结缔组织病相关PCP 67例(1.6%),肾病综合征相关PCP患者16例(0.4%);③4 153例PCP患者中,3 006例提及诊断方法,其中1 785例(59.4%)为临床诊断,915例(30.5%)为染色发现病原体;④2 664例提及治疗,其中2 604例(97.7%)予以磺胺甲恶唑-甲氧苄啶(TMP-SMZ)或TMP-SMZ联合其他药物治疗。结论目前国内PCP患者的基础病主要是HIV感染,器官移植术后,恶性肿瘤,结缔组织病等,该病确诊比较困难,TMP-SMZ是目前治疗PCP的首选药物。Objective To analyze the characteristics of pneumocystis pneumonia（ PCP） in recent five years in China and provide a basis of diagnosis and treatment for PCP. Methods Search the articles relevant PCP published in 2008-- 2012 in data base by Internet. To find out the number of cases, underlying diseases, the diagnosis and treatment of PCP. Results （1） Find out 298 papers which relate to PCP. （2）Total of 4 153 cases of PCP are discovered in the papers,of which HIV -related PCP 3 518 cases （84. 7% ）, organ transplant - related PCP 285 cases （6. 9% ）, cancer - related PCP 94 cases（2. 3% ） ,connective tissue diseases - related PCP 67 cases（ 1.6% ）, nephritic syndrome - related PCP 16 cases （0. 4% ）. （3）Among 4 513 PCP cases, 3 006 cases refered to diagnosis, 915 cases （ 30. 5% ） were diagnosed by staining. （4）Among 4 153 PCP cases, 2664 cases involved to treatment, 2 604 cases（97. 7% ）were treated by trimethoprim - sulfamethoxazole （ TMP - SMZ） or TMP - SMZ combined other medicine. Conclusion Currently, the underlying disease of PCP was mainly HIV infection, the definite diagnosis of PCP is difficult, and TMP - SMZ is effective in PCP patients.
