摘 要:目的探讨新的耐甲氧西林金黄色葡萄球菌(MRSA)染色体基因盒(SCCmec)型别的耐药性和产毒性,进一步明确SCCmec型别的多重耐药模式。方法采用多重聚合酶链反应(PCR)扩增法,对临床分离出的金黄色葡萄球菌菌株进行SCCmec分型及其相关毒力基因耐药性测定。 结果本实验应用9对位点特异性引物对13株MRSA进行了多位点PCR分型测定,共检测到5种不同的测定位点构成图谱,其中3种图谱与已有的ⅢD型、ⅡD型和Ⅳ型相同,2种图谱为未见报道的Ⅱ型和Ⅲ型新亚型。ⅢD型、ⅡD型和Ⅳ型分别发现1株、1株和2株,Ⅱ型新亚型发现6株,Ⅲ型新亚型发现3株,新的SCCmec型别亚型比例较高,占69.2%(913)。新Ⅱ型亚型含有Ⅱ型上游位点B和C,下游位点G,缺乏Ⅱ型下游特异性位点D;新Ⅲ型亚型含有Ⅲ型的上游位点C、E和下游位点F,缺乏下游位点G,多了一个Ⅱ型的下游位点D;其对应菌株的毒力基因检测含有femA、see、etb和tst基因。结论新的SCCmec型别的MRSA,它的基因型与已报道的基因型有所不同,新的菌株同时产3种毒素基因,而且还含有femA基因,故新的菌株不仅是MRSA的高度耐药株,同时产毒性高、耐药性高、抗药谱广,值得引起高度重视和关注。ObjectiveTo explore the antibiotic resistance and toxinogeny of new methicillinresistant Staphylococcus aureus (MRSA) with resistance type SCCmec, and to further clarify the multiple resistance patterns of SCCmec. MethodsMultiplex PCR was used to type the SCCmec of MRSA isolated clinically and to measure the relevant virulence genes resistance. Results In this study, 13 MRSA were detected with nine pairs of specific primer. There were three kinds of patterns like ⅢDtype, ⅡDtype and type Ⅳ. Other two kinds of patterns were probably new subtypes of Ⅲ and Ⅱ. Type ⅢD,ⅡD and Ⅳwere found in 1, 1 and 2. The new subtype of typeⅡwas found 6. The new subtype of type Ⅲ was found 3. High proportion of new SCCmec type subtype accounted for 69.2 % (913).Two new subtype of SCCmec type MRSA were found in this study. The new subtype of typeⅡ contained site B and C in the upstream and site G in the downstream with a lack of specific site D in the downstream. The new subtype of type Ⅲ contained site C and E in the upstream and site F in the downstream with a lack of site G and an additional site D in the downstream. Virulence genes resistance measurements suggested the corresponding strains contain femA, see, etb and tst gene. ConclusionsThe genotype of MRSA with new SCCmec type is different from the previous reports. The new strains can generate three virulence genes and femA gene which have wide and high level resistance and toxinogeny.
