摘 要:目的观察膀胱腔内短期高压力状态对逼尿肌细胞形态及功能的影响。方法将雄性新西兰白兔14只分为2组,每组7只。实验组建立膀胱过度充盈动物模型,保持3h;对照组仅插入导尿管,不施行膀胱过度充盈灌注。所有实验兔均饲养3d后解剖。分别测量膀胱重量并检测逼尿肌功能,对膀胱逼尿肌细胞超微结构进行观察。结果实验组膀胱重量为(3.45±0.36)g,对照组膀胱重量为(4.02±0.47)g,两组比较差异无统计学意义(P〉0.05);实验组在三磷酸腺苷(ATP)溶液、阿托品溶液和无钙液中最大张力较对照组明显增加(P〈0.05);实验组在阿托品溶液、无钙液、高K^+液中最大张力产生速度较对照组明显增加(P〈0.05)。对照组平均每个视野中内质网面积为(0.203±0.097)μm^2,实验组为(1.748±0.772)μm^2,实验组明显大于对照组(P〈0.05);对照组线粒体平均相对密度为1.681±0.219,实验组为1.142±0.085,实验组明显小于对照组(P〈0.05)。结论急性膀胱过度充盈可以导致逼尿肌收缩能力增强,这一变化有其超微结构基础。Objective To evaluate the contractive function and ultrastructure changes of detrusor following acute bladder overdistension. Methods 14 New Zealand white male rabbits were divided into two group, 7 rabbits in each. Overdistension group was induced for 3 hours by forced diuresis and ligation of urethra to volumes 130% of maximum intravesical volume, control group was catheterized with an 8 F catheter but not instillation. The weight of each bladder and function and uhrastructure of detrusor were examined after 3 d. Results The weight of bladder is ( 3.45 ± 0. 36) g in overdistension group and (4. 02 ± 0. 47 ) g in control group. The contractive strength of per 100 mg detrusor of overdistension group increased significantly in adenosine-triphosphate (ATP) solution, atropine solution and Ca^2+ free solution; The contractive velocity increased significantly, in atropine solution, Ca^2+ free solution and high K^ + solusion; The average area of rough endoplactmic reticulum in overdistension group was ( 1. 748 ± 0. 772 ) μm^2 in each unit cytoplasm area versus ( 0. 203 ± 0. 097 ) μm^2 in control group ( P 〈 0. 05 ). The relative density of mitochondrion was 1. 142 ± 0. 085 in overdistension group versus 1. 681 ± 0. 219 in control group (P 〈 0. 05 ). Conclusion Overdisteusion of bladder will increase the constructive function of detrusor, and there are structure foundations of the changes.
