摘 要:目的观察关节腔内注射玻璃酸钠(SH)对膝骨性关节炎(KOA)关节液氧自由基(OFR)含量和滑膜厚度的影响。方法选取KOA患者80例(86膝),分轻、中、重3组,关节腔内注射SH25mg,进行注射前后临床疗效评分。将在4周后仍能抽出关节液KOA(37例,40膝)作为完整监测组,测定注射前后关节液OFR相关因子一氧化氮(NO)、丙二醛(MDA)、超氧化物歧化酶(SOD)含量、超声波检查滑膜厚度。结果轻、中度组注射前与注射4、12周后临床评分变化差异有统计学意义(P〈0.05);完整监测组注射前、后NO含量[(104.84±58.75)、(84.88±57.15)μmol/L]、MDA含量[(6.80±1.21)、(6.02±1.03)μmol/L]均呈明显下降(P〈0.05),而SOD含量[(81.61±14.73)、(85.72±14.95)μmol/L]明显升高(P〈0.05);中度组关节滑膜厚度[(1.58±0.53)mm]较注射前[(1.92±0.71)mm]明显减少(P〈0.05)。结论SH注射KOA可影响患膝关节液OFR水平、减少滑膜厚度、改善临床指征等级评分,更适合治疗轻、中度KOA。Objective To observe the effect of sodium hyaluronate (SH) intra-articular injection on content of oxygen free radical and synovial thickness in patients with knee osteoarthritis (KAO). Methods Eighty cases of KOA (86 knees) were randomly selected, and divided into three groups: mild, moderate and serious. SH (25 rag) was injected intra-articularly. After four weeks can make joint fluid and ultrasonic measurement of thirty-seven patients (40 knees) as a complete monitoring group, measure the contents of nitric oxide, malonic dialdehyde, superoxide dismytasw in joint fluid and synovial thickness before and after four weeks treatment. Results The result of All patients before and after 4 weeks, 12 weeks on mild and moderate group, It is significant difference ( P 〈 0. 05 ) , NO content before and after treatment on complete monitoring group [ ( 104. 84 ± 58.75 ) , 84. 87 ± 57.15 ) μmol/L] , malondialdehyde (MDA) content [ (6. 80 ± 1.21 ) , 6. 02 ± 1.03 ) μmol/Ll , they were significantly decreased (P 〈 0. 05) , superoxide dismutase (SOD) content [ (81.61 ± 14. 73 ) , (85. 72 ± 14. 95) μmol/L] were significantly rise (P 〈0. 05) , synovial thickness [ ( 1.58±0. 53) , ( 1.92 ±0. 71 ) mml in the moderate group significantly decreased ( P 〈 0. 05 ). Conclusion SH treat KOA can affect the level of knee joint fluid oxygen free radical, synovial thickness, clinical indications, the change of the grade, and is suitable for treatment of mild and moderate KOA patients.
