摘  要:目的:从影响糖尿病肾病内皮细胞血管细胞粘附分子-1(VACM-1)的角度,探讨黄芪卫矛合剂对糖尿病肾病的治疗作用和机制。方法:采用单侧肾切除并链脲佐菌素尾静脉注射建立大鼠糖尿病肾病模型,随机分为假手术组、糖尿病肾病组、糖尿病肾病+黄芪卫矛合剂组和糖尿病肾病+氯沙坦钾组。观察12周末各组大鼠体重、尿量、饮水量、尿蛋白排泄率、血糖、糖化血蛋白、血肌酐、尿素氮、Ⅳ型胶原蛋白、层粘连蛋白的变化。用免疫组织化学染色法检测肾小球VACM-1和α-SMA的表达。结果:黄芪卫矛合剂按40g(生药)/kg给药和氯沙坦钾按16mg/kg给药可减少模型组大鼠饮水量、尿蛋白排泄率和层粘连蛋白,降低VACM-1和α-SMA在肾小球内表达。黄芪卫矛合剂还可减少模型组大鼠尿量,降低血糖、糖化血红蛋白、血脂,在减少尿量和降低糖化血红蛋白方面优于氯沙坦钾组。结论:黄芪卫矛合剂可减少糖尿病肾病大鼠肾组织中VACM-1表达,抑制内皮细胞的表型转化,保护肾脏内皮细胞,从而对糖尿病肾病大鼠肾组织有保护作用。Objective: The mechanism of Huangqi-Weimao mixture in the treatment of diabetic nephropathy from the effect of endothelial cell VACM-1. Methods: Diabetic nephropathy model in rats was established by the unilateral kidney and streptozotocin intravenously, after the success of modeling, the rats were randomly divided into sham operation group, diabetic nephropathy group, diabetic + hqwm mixture group and diabetic nephropathy + losartan potassium group, continued for 12 weeks. The weight, urine volume, water intake, urine protein excretion, blood glucose, glycosylatod serum protein, serum creatinine, urea nitrogen, collagen IV and laminin changes were observed, and so the immunohistochemical staining of glomerular VACM-1 and α -SMA expression. Results: The Huangqi-Weimao mixture which was given as 40mg( Crude drug )/kg and losartan which was given as 16mg/kg can reduce the amount of water, urinary protein excretion rate and laminin in the model the rats ( P 〈 0.05, P 〈 0.01 ), hqwm mixture group can also reduce the model group rats urine volume, decrease blood glucose, glycosylated hemoglobin, blood lipids (P 〈0.05, P 〈0.01 ), in reducing the urine volume and reduce glycated hemoglobin is better than that of losartan potassium group ( P 〈 0.05). VACM-1 and α -SMA expression in model rats were signiflcantly higher than those in the sham operation group ( P 〈 0. 01 ), the two treatment group can significantly reduce the expression of VACM-1 and α -SMA in glomeruli (P 〈 0.01 ). Conclusion: Huangqi-Weimao mixture can reduce the expression of VACM-1 in renal tissue of diabetic rats, which has a protective effect on renal tissue of diabetic nephropathy rats.
