部分肠外营养对腹部结核病支持作用的探讨

摘　　要：目的　探讨含脂肪乳剂的双能源部分肠外营养 (PPN )对腹部结核病营养状态的影响。方法　将 2 1例肠结核、36例结核性腹膜炎患者随机分为部分肠外营养组 (PPN组 ,30例 )与对照组 ( 2 7例 ) ,按组别给予含或不含脂肪乳剂的PPN支持治疗 2周 ;PPN前后分别作人体测量及血清白蛋白 (ALB)、前白蛋白 (PA)、纤维结合蛋白 (Fn)与铁蛋白 (Tf)测定。结果　经PPN两周后 ,PPN组理想体重百分率 (BW %IBW )与理想三头肌皮褶厚度百分率 (TSF %ITSF)显著增加 (P <0 .0 5 ) ,血清PA、Fn与Tf明显升高 (P <0 .0 5 ) ;对照组均无明显变化。Objective To evaluate the effect of partial parenteral nutrition(PPN) with fat emulsion on nutritional status in patients with abdominal tuberculosis.Methods 21 patients with intestinal tuberculosis and 36 ones with tuberculous peritonitis were randomly divided into PPN group (30 cases) and control group (27 cases);patients in PPN group were given PPN with fat emulsion for two weeks.Their anthropometric measure was made and serum albumin(ALB),prealbumin(PA),fibronectin(Fn) and transferrin (Tf) were examined respectively before and after PPN.Results Having been given PPN for two weeks,percentages of body weight to ideal body weight (BW%IBW) and triceps skin fold to ideal triceps skin fold(TSF% ITSF) were increased,serum PA,Fn and Tf were raised remarkably in PPN group (P<0.05);all of nutritional parameters were changed unremarkally in control group (P> 0.05 ).Conclusion Administration PPN with fat emulsion for patients with adominal tuberculosis may make more improvement on nutritional status than PPN without fat emulsion.
