肌钙蛋白T和CRP在急性心肌梗死中的应用价值

摘　　要：目的探讨血清C反应蛋白(CRP)和心肌肌钙蛋白T(cTnT)在诊治急性心肌梗死(AMI)患者中的临床应用价值和意义。方法选择67例AMI患者(观察组)、51例非心血管疾病患者(对照组)及健康体检人群56例(健康对照组),分别测定CRP和cTnT水平并进行比较。结果观察组CRP及cTnT检测阳性率分别为82%和84%,对照组分别为6%和0,健康对照组分别为5%和0,观察组与对照组和健康对照组比较有显著性差异(P均<0.01),对照组与健康对照组比较无显著性差异(P>0.05)。观察组cTnT与CRP水平显著正相关(r=0.982,P<0.01)。观察组CRP、cTnT单独检测时,其敏感度、特异度分别为82%,90%和84%,100%;联合应用两者后其敏感度为88%,特异度91%。结论 cTnl与CRP对AMI的诊断及早期诊断具有重要的临床应用价值,值得临床推广应用。Objective It is to approach the clinical application Values and meaning of cardiac troponin T （cTnT） and C - reactive protein （CRP） in patients with acute myocardial infarction （AMI）. Methods The plasma cTnT and CRP were detected and compared in 67 patients with AMI （observation group）, 51 patients excluding cardiovascular diseases （control group） and 56 healthy people （healthy control group）. Results The positive rate of cTnT and CRP was 82% and 84% in observation group, 6% and 0 in control group, 5% and 0 in healthy control group. There was significant difference between observation group and control group or healthy control group （ all P 〈 0. 01 ） , there was no significant difference between control group and healthy control group （ P 〉 0.05 ）. There was significant positive correlation between cTnT and CRP in observation group （ r = 0. 982, P 〈 0.01 ）. For the separate test of cTnT and CRP in observation group, sensitivity were 82% and 90% respectively, specificity were 84% and 100% respectively. The specificity and sensitivity of the two combined application were 91% and 88% respectively. Conclusion The detections of cTnT and CRP have very important clinical value in diagnosis and early diag- nosis of AMI, it is worth for clinical use application.
